The participants submit:

1. Inventory of the documents presented, contained in the package, signed by the participant (in the original) or by his duly accredited representative (free text).

2. The application form includes the following documents:

2.1. Single European Public Procurement Document (EDES) in accordance with the requirements of Art. 67 of the PPA and conditions

Unified European procurement document for the participant in accordance with the requirements of the law and the conditions of the contracting authority, and where applicable - OED for PL by a power of attorney representing the participant or a legal entity within its control or management body; for each of the participants in the association, which is not a legal entity; for each subcontractor and for any person whoseresources will be engaged in the execution of the contract (Annex No 4) - mandatory in electronic form.
