The subject of filling of the order is to provide health services - the sightings of bodies who died in the Central Bohemian Region in accordance with Section 84 (2) of the Charter. b) Act No. 372/2011 Coll., on health services and on the conditions for their provision, in the wording of later regulations

The subject of filling the order is fromassurance of health services - the view of bodies died in the Central Bohemian Region in accordance with Section 84 (2) of the Charter. b) Act No. 372/2011 Coll., on health services and the conditions for their provision, as amended (hereinafter referred to as the "Health Services Act"), in particular: - - to inspect the bodies of deceased persons outside the health care facilities in the Central Bohemia Region continuously, ie 7 days in youday and 24 hours a day, unless this is a procedure under Section 84 (2). (a), (c) and (d) of the Health Services Act, - to ensure that the body of a deceased person is physically carried out by doctors in accordance with Section 84 (2) of the Charter. b) Act on Health Services, exclusively after the call of the Health Care Service Operations Center, without undue delay, within 120 minutes of receiving the calls,

- To operate a telephone number that will be available on a 24-hour basis will be published and will be available to the Health Service of the Central Bohemian Region for receiving a call to the line number 155 or to a single European 112 call number in accordance with the provisions of Section 110, paragraph 1, letter. b) The Act on Health Services.

Following the entry into force of the Agreement, (1) and (2) of the Act

The Contracting Authority requests the following: - The method of proving the above-limitation regime - The selected contractor shall submit the documents referred to below in the procedure pursuant to Section 122, paragraph 3 . (a) the filing of a commercial listing or other similarly to the filing, if another legal provision required by such records is required;(B) submitting proof that the supplier is entitled to engage in business to the extent of the subject matter of the public offer, unless otherwise required by law;

- Valuable Decision to grant a right to provide health services in the territories of the whole of Central Bohemia of the Region,

- Original Declaration of Facts, Techniques and Personal SecurityHealth Service Provided - View of Bodies Deceased for the Central Bohemian Region
