CPV Description: Medical practice services. The service of preventive and diagnostic examinations in the field of occupational medicine in the years 2019-2020. Reference Number: MED / 573/2018. The subject of the order is collateral in 2019 and in 2020 for 2 WOG and JW / Institution remaining on the supply of 2 GWP services in the scope of: - The subject of the orders is collateral in 2019 and in 2020, for 2 WOG and JW / Institution-remaining in the supply of 2 WOG services in the scope of: - preventive and diagnostic examinations in the field of occupational medicine for professional soldiers / staff / as well as employees of the army and soldiers of the National Reserve Forces,

- making annual job reviews,

- and participation in committee work OHS.

Having an entry in the register of entities carrying out medical activities conducted bywith the territorially competent administrative body, in accordance with the Act of 15.4.2011 on medical activity (ie, Journal of Laws of 2018, item 160 with later amendments, changes)

1. Due to the limitation of the number of signs, statements and documents regarding the lack of grounds for exclusion have been specified in Chapter VI of the Terms of Reference.

2. A bid bond is established in the amount of: - for task No. 1 44 000.00 PLN (in words: forty-four thousand zlotys 00/100), for task no. 2 PLN 2, 000.00 (in words: two thousand zlotys 00/100), for task no. PLN 3, 500.00 (in words: five thousand zlotys 00 / 100), for task No. 4 1 000.00 PLN (in words: one thousand zlotys 00/100).

3. The Contracting Authority provides for the possibility referred to in art. 24aa paragraph 1 Public Procurement Laws

4. The Contractor may entrust, pursuant to art.36a paragraph 1 of the Public Procurement Law, execution of a part of the order

Subcontractors. The ordering party requires the contractor to indicate the part of the orders, the execution of which he intends to entrust to subcontractors and the submission of subcontractors by the contractor (Article 36b paragraph 1 of the PPL Act). The Contractor is obliged to attach a statement valid at the day of submitting bids that the contractor:

a) Is not subject to exclusion; b) meets the conditions-participate in the proceedings in the form of a single European order document (EED).

6. The contractor will be excluded from the contract award procedure in respect of which any of the circumstances referred to in Article 24 sec. 1 item 12 - 23 and art. 24 sec. 5 Public Procurement Laws

7. The Contracting Authority provides for the provision of the basic contractor for the basic order, including the orderch repeated similar services included in art. 67 sec. 1 point 6 of the PPL Act consisting in securing medical services in the field of occupational medicine and supervision of dispensation. The value of the above the service was included in the estimated value of the subject of the order.

8. The contractor in a single European order document may confine himself to completing only the ALFA section in Part IV
