PAAPD1033659 Framework Agreement is to "Providing medical oxygen produced on-site." The acquisition is based on the necessity essay no. 124 / 10.04.2018, approved by the management unit.

Main Site: County Hospital Satu Mare.

The framework agreement covers"Provide on-site medical oxygen supply."

A framework agreement will be concluded for a period of 24 months. 12 consecutive contracts

The estimated amounts of the framework agreement and the subsequent contracts are set out in Annex 1 to the tender specifications.the following trades will be completed according to the needs of the contracting authority, the quantities being estimated and are found in the Annex to the tender book.

The value of the largest subsequent 12 month contract is RON 718, 080.00, excluding VAT. a large subsequent 1-month contract is RON 59, 840.00, excluding VAT.

Tenderers, Associate Tenderers, Supporting Subs and Substhe contractors must not find themselves in the situations provided by art. 164, 165, 167 of Law no. 98 / 2016.

The failure of the DUAE with the offer, attracts the rejection of the offer as unacceptable.

The way to prove the fulfillment of the requirement: To complete the DUAE by the economic operators participating in the award procedure (including any third parties and subcontractors)their situation and signed accordingly. The DUAE Form shall be signed with the extended electronic signature.

The supporting documents proving the fulfillment of the assumed completion by the DUAE shall be submitted, at the request of the contracting authority, only by the tenderer placed on the first place in the intermediate rankings completed at the evaluation of the bids.-the supporting documents proving the fulfillment of the declarations stated in the DUAE may be: - - certified certificates regarding the lack of overdue debts, regarding the payment of taxes, taxes or contributions to the consolidated general budget (local budget, state budget, etc.) a legible copy showing that the tenderer has no overdue debts at the time of filing, -or the value of such debts falls within the value thresholds provided for in art. 166 par. (2) of the Law no. 98/2016 at the time of submission, - the criminal record of the economic operator and the members of the management, management or supervisory body of that economic operator or of those having the power of representation, decision-making or control within it, as reout of the certifying certificate issued by the ONRC / constitutive document, - if necessary, documents demonstrating that the economic operator may benefit from the derogations provided in art. 166 par.
