VIII a. Necessary documents to be submitted by the contractor.

1. The offer (Annex No. 1 to the ToR) together with the assortment price form constituting Annex No. 1 to the offer.

2. The Single European Order Document, the so-called EAT (Annex No. 3 to the ToR).

3 Original power of attorney (powers of attorney) or a copy certified by a notary public, provided thatthe offer will be signed by a proxy [required in particular when the offer is submitted by jointly acting entities (consortium) and the offer is not signed by all members of the consortium]. In this case, the original power of attorney or a copy certified by a notary public must be submitted together with the offer in electronic form (separate file) with a qualified electronic signatureby the person granting the power of attorney or notary, and then together with the files constituting the offer, they should be compressed into one archive file in the * .zip format.

4. Deposit.

5. Documents and statements below listed current as at the date of submission, which the Contractor will be obliged to submit at the request of the Employer within a specified time limit, not shorter than 10 days - applies to the contractor whose offer isthe highest rated.

1). Information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids (applies to members of the Management Board, proxies, members of the supervisory board, if appointed, companies)

2). Contractor`s declarations of no final judgment or final judgment against himadministrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the event of such judgment or documentary evidence confirming the payment of such receivables together with any interest or fines, or concluding a binding agreement on the repayment of these receivables.

3) Contractor`s declaration of non-arrearspayment of local taxes and fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2016, item 716).

6. Documents submitted by the EntityForeign.

1) If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, he shall submit a document or documents confirming the circumstances referred to in art. 25 paragraph 1 point 3) of the Public Procurement Law issued in the country in which it has its registered office or place of residence:

a) information from the relevant register or, in the absence of such register, another equivalent document issued bysquint the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act;

b) its liquidation has not been opened or bankruptcy has been announced.
