The subject of public procurement is performed-upgrade of the MR scanner Philips version Achieva 1.5T

The subject of the public procurement is to dismantle and remove the existing equipment or its replaced parts, which will be disqualified by the contracting authority in connection with the fulfillment of the object ..

The subject of the public procurement is to carry out the instruction of the contractor's employees in the operation and routine maintenance of an upgraded MR scanner in a sense and in accordance-- 61 of Act No. 268/2014 Coll., on Medical Devices and on the Amendment to the Act No. 634/2004 Coll., on Administrative Fees, as amended.- The subject of public procurement is to provide guarantees for the quality and implementation of the comprehensive guarantee upgraded MR scanner service, including all its present and accessory, for 1 year after the upgrade of the MR scanner. Parts formanual service are servicing and revisions in the sense and in accordance with Chapter IX of the Act on Sickness

The supplier under Section 77 (1) of the ZZVZ proves the fulfillment of his / her professional competence in relation to the Czech Republic by submitting a commercial listing or similarly, other law required to be entered in such records.

The Contracting Authority in accordance with Section 77 (2) of the Decree. a) APHD requires the supplier to submitThe Contracting Authority requires the Contractor to submit the appropriate trade license or license, at least for the trades of the Manufactures, Trade and Services not listed in Appendices 1 to 3 of the Trades Licensing Act or another similar trade corresponding to the subject of the public procurementin accordance with Section 77 (2) of the Act. (c) the law requires the Contractor to furnish proof that he is professionally qualified or has a person who, through professional competence, ensures that he / she is required to fulfill the public procurements by requiring other legal provisions.

The Contracting Authority requires the Contractor to submit: on fulfillment of the reporting duty in the sense of - 26 of Act No. 268/ 2014 Coll., On Medical Devices and on the Amendment to Act No. 634/2004 Coll., On Administrative Fees, as amended, in the wording of later regulations (hereinafter referred to as the "Medical Devices Act"), for the production of purchased medical devices (if a participant of the invigorating management is the manufacturer of the medical devices purchased under the Medical Devices Act) or for import-or distribution of purchased medical devices (if the entrant is controlled by an importer or a distributor of purchased medical devices in accordance with the Medical Devices Act), b) a document certifying the fulfillment of the reporting duty pursuant to Section 26 of the Medical Devices Act for the servicing of medical devices according to the Medical Devices Act)-on fulfillment of the notification obligation, the confirmation of fulfillment of the reporting duty is issued by the State Control Body (SLA) or other similar document, which clearly proves that the reporting obligation was fulfilled by the entrant of the applicant for the mediated medical devices. Section 73 of the APIA does not require proof of economic qualification pursuant to - 78 of the APIA.

Zadavatel allow all vendors to visit the premises. The completion of the site inspection is given in the documentation for the adjudication.
