Purchase of equipment for the endoscopy laboratory included in I-XI packages for the needs of SP ZOZ in Mońki

1. The subject of the order is the delivery, installation and servicing of brand-new devicesmedical episodes included in I-XI packages for the needs of the Endoscopy Laboratory of SP ZOZ in Mońki, ie:

Each offer must be secured with a security deposit of 19, 960.00 PL (for a total of 11 packages). Other requirements regarding the bid bond are included in Chapter XXI of the ToR.

II. The offer should be submitted at the headquarters of the Ordering Party: Sekretariat Samoa brave Public Health Care Center in Mońki, ul. Al. Niepodległości 9, 19-100 Mońki (the building of the District Clinic in Mońki).

III. The Awarding Entity does not provide for the selection of the most advantageous offer with the use of an electronic auction.

IV. The Employer does not provide for the conclusion of a framework agreement and the use of electronic auction and the establishment of a dynamic purchasing system V. Purchaserdoes not provide for the ordering referred to in art. 67 sec. 1 point 6 and 7 or art. 134 sec. 6 point 3 Pzp

VI. The grounds for exclusion are specified in Chapter XI of the ToR.

1. The Ordering Party will exclude from the Contractor's proceedings in the event of occurrence of circumstances referred to in:

1.1 art. 24 sec. 1 point 12-23 of the Act on Public Procurement Law,

1.2. Article 24 paragraph 5 points 1 and 8 of the Public Procurement Lawicznych.

2. Contractor who is subject to exclusion pursuant to art. 24 sec. 1 point 13 and 14 and 16-20 or paragraph 5, may provide evidence that the measures taken by him are sufficient to demonstrate its reliability, in particular to prove the remedy for damage caused by a crime or fiscal offense, financial compensation for the harm suffered or remedied, exhaustedclarifying the facts and cooperation with law enforcement agencies and taking specific technical, organizational and human resources measures that are appropriate to prevent further offenses or fiscal offenses or improper conduct of the contractor. The provision of the first sentence shall not apply if the contractor, being a collective entity, has been declared final-the court ruling prohibits to apply for the order and the period of validity of the prohibition specified in that judgment has not expired.

3. The Contractor is not subject to exclusion, if the Ordering Party, considering the weight and special circumstances of the Contractor's action, deems sufficient evidence presented on the basis of item 2.

4. In cases referred to in art. 24 sec. 1 point 19, before exclusionawcy, the Employer will provide this Contractor the opportunity to prove that his participation in the preparation of the procurement procedure will not distort competition.

5. The tender of an excluded Contractor shall be considered rejected.

6. The Ordering Party may exclude the Contractor at any stage of the proceedings for the award of the order.

7. To confirm that you are not subject to exclusion from the proceedings, the Contractor is obligedmust submit with the offer an up-to-date statement in the scope indicated by the Ordering Party in the Contract Notice and SIWZ in the form of a Uniform European Order Document, hereinafter referred to as "Single Document." VII List of statements or documents confirming compliance with the conditions of participation in the proceedings and no grounds for exclusion have been specified in Chapter XV, XVII and XVIII SIWZ.

VIII. The description of how to assess compliance with the conditions for participation in the proceedings and the lack of grounds for exclusion is set out in Chapter XII of the ToR.

IX. Other documents that the contractor must attach together with the offer are specified in Chapter XVI of the ToR.

X. The contracting authority provides for the possibility of applying the procedure indicated in Article 24aa, i.e. first making an assessment of the offers and then examining whether the contractor whoego the offer was rated as the most advantageous, it is not subject to exclusion and meets the conditions for participation in the proceedings.

XI. The Administrator's information clause regarding the GDP is included in Chapter XXXVII of the ToR.
