Subject of this order is to conduct didactic classes on specialization courses for doctors participating in the projecttitled: "The development of specialist education of doctors in areas relevant to the epidemiological and demographic needs of the country" co-financed by the European Union from the European Social Fund under the Operational Program Knowledge Education Development, planned for implementation by clinics and CMKP facilities in 2020. the order is divided into 9 lotsthe detailed description of the subject of the contract in relation to individual parts of the contract is Annex 1A and Annex 1B to the ToR. The Employer allows the submission of bids for any number of lots.

Lot 1: Course provider: Department of Pathomorphology CMKP; Course title: "Oncological histology, basic scope"; Topics of teaching: WHO classification of the most common breast cancers, the use of special tests (immunohistochemistry, electron microscopy and molecular) in the classification and diagnosis of differential. Predictive and predictive factors. Lectures 5h, Microscopic exercises in the field of breast cancer 3h .; Number of didactic hours planned / reserved for conducting didactic classes perone course (1 teaching hour = 45 minutes): 8; Number of course editions: 1; Total number of didactic hours planned / reserved for conducting didactic classes on all editions of the course (1 teaching hour = 45 min): 8; Value of the contract in PLN: 3, 600.00

1. This contract is co-financed by the European Union from the European Social Fund under the Operac ProgramKnowledge Knowledge Education Development 2014 2020 under Priority Axis V Support for the health area, Measure 5.4 Professional competences and qualifications of medical staff and is implemented for the needs of the project entitled: "Development of specialist education of doctors in areas relevant to the epidemiological and demographic needs of the country" .

2. Criteria for the evaluation of bids:

1) Price 60%;

2) Experiencedidactic activity of the person appointed to conduct classes 20%;

3) Clinical / professional experience of the person appointed to conduct classes 20%;

A detailed description of the criteria and how to evaluate the offers is given in chapter X of the ToR.

3. The period of validity of the contract referred to in section II point 2.7 in relation to individual parts of the contract should read:

1) Individualparts of the order will be implemented within the time of the course of the course, within which the didactic classes that are the subject of the given part of the order are to be conducted, in accordance with the requirements set out in Annex No. 1A and Annex No. 1B to the ToR.

2) The Ordering Party requires the order to be completed by 31.12 .2020.

3) The Employer allows the possibility of canceling the course or changing the planthe deadline for its implementation, if the number of participants in the course is less than 50% of the number of participants planned for a given course. In this case, the course completion date may be changed to no later than 31 March of the calendar year immediately following the year in which the course was originally planned.

4.
