Delivery of medical devices in 2021-2022

Delivery of medical devices for the needs of the contracting authority under 186 lots, detailed in the "technical specifications" of the OP documentation.

Main Site: Structural unit of "KOTS- Stara Zagora "- Stara Zagora city, 15" Dr. T. Stoyanovich "Str., Specified in the application.

Syringes - three-component in type and quantity according to"Specification of medical devices" from the OP documentation. The requirements for the device are specified in the "Specification of medical devices"

The participants must have the right to wholesale the medical devices under the terms and conditions of the Medical Devices Act (MIA) or to be among the persons referred to in Art. or Article 28 of the Media and have the right to carry out the transportation of sources of ionizing radiationof the Nuclear Energy Safe Use Act (ASUNE) - in case the tenderer offers products that fall within the scope of the law (where applicable). of the EDES, and upon signing the OP contract it is proved with the following documents:

1. On the wholesale medical lawMass media products - shall be evidenced by a valid wholesale trade permit for medical devices under Article 79, paragraph 2 of the Mass Media issued under the terms and conditions of this Act or shall be evidenced by a document certifying the wholesale trade in medical devices, issued by a competent authority of an EU Member State, a State Party to the Agreement on the European Economic Area or to the ConfederationSwitzerland pursuant to Article 77, Paragraph 1 of the Media.

2. Concerning the legal personality under Article 27 and Article 28 of the Media - with a certificate of registration under Article 27 or Article 28 of the Media, issued pursuant to the terms and conditions of this Act. Concerning the right to transport ionizing radiation sources under the ASUNE - shall be proved by a valid license for the transport of radioactive substances issued under the terms and conditions of the ASUNE - Art. 58, para 1, item 4 inconnection with Article 56, paragraph 1 of the CC

The participants - foreign persons should be entered in similar registers of the mentioned in the previous points according to the legislation of the Member State in which they are registered, and at the conclusion of the contract should submit the documents under item 1, item 2 and item 3.

When submitting tenders, they shall certify the absence of these grounds by completing the applicable fields of Part III of the OEDOP, and at the conclusion of the contractand awarding the OP with the following documents: under item 1 - with a criminal record issued by the District Court; under item 2 of the relevant competent authority; under item 3 - with a unit from the municipality at the seat of the participant; under item 4; item 5; v.7; v.8; p. 9 and p. 10-with a declaration according to a sample of the OP documentation.
