This order is for periodic deliveries of extracurricular supplies of the body from toxic substances at" UMBAL "Alexandrovska" EAD for 2 separate items: - Lot # 1 "Consumables compatible with the deviceOmni, manufactured by B. Braun ", and

Main Site:" Alexandrovska "UMBAL, CAIL and the Nephrology Clinic.

Periodic deliveries of extracorporeal supplies for the body of toxic substances compatible with the Omni apparatus, manufactured by B. Brown.

The types, characteristics and quantities of medical supplies are detailed in the technical specification attached to the documentation

The quantities indicated in the technical specification are estimated and depend on the volume of activity of the contracting authority.

Each participant is entitled to participate for one or both lots. 1 application for participation is allowed.

When submitting the offer, participants declare the presence of the abovethese are the circumstances below, by filling in the necessary information in the relevant fields in the EDES Part IV "Selection Criteria". para 1, item 2 of the Public Procurement Act

Eligibility criterion:

1. Permission for wholesale trade in medical devices, issued under the procedure of the Medical Devices Act.The eligibility is described in Part IV "Selection Criteria", letter A "Eligibility", specifying the authorization number. medical devices - a copy certified by the participant

The client does not place requirements on the economic and financial status of the participants in thethe procedure

The assignor removes from participation participants: for whom the circumstances under Art. 54, para. 1, items 1-7 and Art. 55, para. 1, item 1 and item 4 of the Public Procurement Act, Art. 107 of the Public Procurement Act; have no right to participate in public procurement on the grounds of art. 3, item 8 of ZIFODRUPDRLCTDS; availability of circumstances under Art. 69 of the Anti-Corruption and Forfeiture of Illegal Acquisition Act; which after nthe contracting authority's invitation does not extend the period of validity of the tender. The circumstances mentioned above are detailed in the documentation for participation. If there is a ground for removal from the procedure, the relevant participant may provide evidence that he or she has taken measures that guarantee its reliability in accordance with Art. 56, para. 1 of the PPL. Declaring the presence or absence of a justified hand the removal is effected by the appropriate marking in Part III of the annexed OED model documentation. Prior to the conclusion of the public procurement contract, the successful tenderer shall submit the documents referred to in Art. 58 of the PPA, to prove the lack of grounds for removal, except when the relevant circumstances are accessible through a public free register or information orand access to it is provided by the competent authority of the contracting authority ex officio.

Upon conclusion of the contract, the tenderer designated as a contractor shall submit a performance guarantee of 2% of the contract value excluding VAT.

The performance guarantee shall be presented in one of the following forms:

1. cash amount;

2. bank guarantee;

3. insurance that ensures the performance through coverage

If the contractor designated as the contractor chooses the form of the performance guarantee.

In case the selected contractor chooses to present a performance guarantee by monetary amount, the latter is transferred to the following account of the contracting authority:
