Medium Supply Consumption Supplies for Medtronic Paradigm and Accu-Chek Spirit Insulin Pump

Compatible supply agreement for Medtronic Paradigm and Accu-Chek Spirit insulin pumps. The minimum and maximum quantities of batches and the value of the largest subsequent contract per lot are prevalentin the Task Force. Subsequent contracts will be concluded on the basis of framework supply agreements based on the budget funds allocated to this destination, the largest subsequent contract for each lot, estimating that it will be monthly or or any period covered by the available budget. : Str. Ion Movila no. 5-7, sector 2 Pharmacy.

Consumables compatible with the pumpof the Paradigm Medtronic insulin.

The minimum and maximum amounts of the framework agreement are set out in the Task Force. The value of the largest subsequent contract is RON 46 119.00. 1:

Mode of Performance The economic operator (Leader, Associate, Subcontractor) will completein the DUAE form of the awarding documentation.

The inclusion in the situation provided in art. 164 of Law no. 98/2016 regarding the public procurement attracts the exclusion of the bidder from the procedure applied for awarding the public acquisition contract. 2:

Declaration on non-compliance with the provisions of art. 165 of Law no. 98/2016 on public procurementboring with art. 166 par. Means of performance The economic operator (leader, associate, subcontractor) will fill in the corresponding requirement in the DUAE form in the awarding documentation. 165 of Law no. 98/2016 regarding the public procurement attracts the exclusion of the bidder from the procedure applied for awarding the public acquisition contract. 3:

Declaration on non-compliance with the provisions of art. 167 of the Lawno. 98/2016 on public procurement. The contracting authority excludes from the award procedure any economic operator who is in any of the following situations: (a) has violated the obligations established under art. 51, and the contracting authority can demonstrate this by any appropriate means of proof, such as decisions by the competent authorities, which are still to be found(c) has committed a serious professional misconduct that questions its integrity, and the contracting authority can demonstrate this by: (a) (b) is in the process of insolvency, liquidation, judicial supervision or cessation of activity; any appropriate evidence, such as a decision by a court or administrative authority, ) the contracting authority has sufficient reasonable indications / concrete information to consider that the economic operator has entered into agreements with other economic operators aimed at distorting competition within or in connection with the procedure in question; (e) is in a situation of conflict of interest interests in or in connection with the procedure in question, and this situation can not be remedied(f) the previous economic operator`s participation in the preparation of the award procedure has led to a distortion of competition, and this situation can not be remedied by other less stringent measures; (g) the economic operator seriously or repeatedly breached its main obligations under a procurement contract(h) the economic operator has been guilty of misrepresentation in the performance of the contract, the payment of damages or other comparable penalties; falsely in the content of the information submitted at the request of the contracting authority for the purpose of verifying athe reasons for exclusion or the fulfillment of the qualification and selection criteria, did not submit this information or is not able to submit any documents on www.e-licitatie.ro

In order to meet the qualification criteria, the bidder will fill in the DUAE available in electronic format, following AC to request doc. only supporting the winning bidder in the rankingafter the application of the award criterion. The failure of DUAE with the offer (including the associate / third party supporter) entails rejecting it as unacceptable. In the case of equal pricing offers, the penalty payment will be made by submitting a request for clarification from the CA, to which op. ec. will transmit the new prices, which will be prrepresented by SICAP. The DUAE will specify the lots (lot number) for which the economic operator submits the offer. The bidders will be required to fill in the price in RON, without VAT, for each lot, taking into account the minimum and maximum amounts estimated over the entire validity period of the framework agreement. The price posted in SICAP is the price in RON without VAT for maximy lot. The offer that does not fall within the maximum estimated value in RON without VAT will be declared unacceptable. At the time of submitting bids, DUAE, is accepted as preliminary evidence instead of certificates issued by public authorities or by third parties that confirm as ops. ec. meets all the qualification requirements in the data sheet. Bidders will fill in the "Product Identity Form (Producer Details Table), " attached to the Data Sheet in the "Forms" section. The specified form will be submitted by the bidderand within the Technical Proposal. The document will be signed with the extended electronic signature and will be posted to the SICAP.

The bidders will fill in the form "Declaration regarding the acceptance of the framework agreement model and the subsequent contract contract as well as the date of its signing", attached to the Data sheet in the " Forms ", by the deadline for submitting the offer (will be posted in SICAP) Formspecified will be submitted by the tenderers signed with the extended electronic signature and will be posted to the SICAP.

The framework agreement may be modified under the conditions of Art. 221 of Law 98/2016 with subsequent amendments and completions
