The subject of the order is the service of preparation, delivery and distribution of meals

The subject of the order is the service of preparation, delivery and distribution of meals in the facility at ul. bumblebeeslowiczkiej 28 in Wrocław. The detailed description of the subject of the order is specified in the following annexes to the Terms of Reference:

* Annexes No. 1 Bid Form,

* Annex No. 4 - Agreement Model,

* Annex No. 5 - Description of the subject of the order.

1) Possession of the current decision territorial State Sanitary Inspector, confirming the unconditional approval of a place where food is prepared, in relation to meeting the relevant requirements of food law in the scope of: production of ready-made foodstuffs from raw material to ready-made food, trade in beverages and permanent foodstuffs in unit packaging (applies to the place where meals will be prepared as part of the service related to the contract, valid throughout the entire period of service services);

2) Having currentj decision of the territorially competent State Sanitary Inspector that allows food transport vehicles (applies to vehicles that will be used as part of the service related to the contract, valid for the entire service period).

3) Managing the implemented HACCP system at the parent plant, at the place where the meals are prepared, and include the object accepted from the Ordering Party (room) to the said HACCP system.

1. Due to the limitation of the number of signs, statements and documents regarding the lack of grounds for exclusion

They were specified in Chapter VI of the Specification of Essential Terms of Orders.

2. A bid bond is established in the amount of PLN 15, 000.00 (in words: fifteen thousand zlotys 00/100)

3. The Contracting Authority provides for the possibility referred to in art. 24aa paragraph 1 Public Procurement Laws

4. The Contractor may entrust, pursuant to art. 36a paragraph 1 of the PZP Act, the execution of a subcontractor's part of the order. The ordering party requires the contractor to indicate the part of the orders, the execution of which he intends to entrust to subcontractors and the submission of subcontractors by the contractor (Article 36b paragraph 1 of the Public Procurement Law).

5. The Contractor is obliged to attach a statement, valid as of the day of submitting bids, that the contractor:

a) Is not subject to exclusion; b) meets the conditions for participation in the proceedings in the form of a single European Order Document (EIN).

6. The contractor will be excluded from the contract award procedure in respect of which any of the circumstances referred to in Article 24 sec. 1 item 12 - 23 and art. 24 sec. 5 Public Procurement Laws

7. The contracting authority provides for the award of the contractor to the contractbasic, orders for additional preparation, delivery and distribution services included in art. 67 sec. 1 point 6 of the Public Procurement Law - 8. The contractor in a single European order document may limit himself to completing only the ALFA section in the

IV section. 9. The ordering party, prior to the order being awarded, will call the contractor whose offer has been rated the most, to submit in the designated one, e shorter than 10 days, the current date as of the date of submission of the following declarations or documents listed in Section VI item 7, sub-point a) -m) of the ToR.
