The subject of the orders is the sale and delivery of a confocal Raman microscope, enabling 3D Raman imaging together with other devices making up the system, for WIMiC - Kc-zp.272-419 / 18. Item description orders in accordance from point III.1. Terms of Reference.

1. For orderingia Contractors who are not subject to exclusion under Article 24 para. 1 point 12-23 and art. 24 sec. 5 points 1 of the Public Procurement Law

2. Documents submitted to the ordering party's request:

a) excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion-based on art. 24 sec. 5 point 1 of the Act;

b) information from the National Criminal Record, to the extent specified in art. 24 sec. 1 points 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for the submission of bids;

c) the contractor's statement that the contractor has not been the subject of a final court decision or a final administrative decision on default, oppats or social or health insurance premiums or, in the event of such a judgment or decision being issued, documents confirming payment of such claims together with any interest or fines or conclusion of a binding agreement on repayments; d) declaring the contractor about the lack of a decision against him as a preventive measure for the prohibition of applying for-public orders

3. Foreign contractors:

If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point a) and b) respectively: - a document or documents issued in the country in which the contractor has its registered office or place of residence, confirming respectively that its liquidation has not been opened or that the bankruptcy has been declaredthose issued not earlier than 6 months before the deadline for submission of tenders, - information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence or place residence has a person who is concerned with information or a document in the scope specifiedin art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders.

The Contracting Party does not describe, does not set a specific condition in this respect.

1. The offer must be secured by a bid bond in the amount of PLN 8, 000.00 (in words: eight thousand PLN 00/100)

2. The bid bond should be submitted before the deadline for submitting bids.

3. The bid bond can be brought inone or more of the following forms:

• money:

• bank guarantees, or guarantees of the Social Credit and Savings Fund, with that the surety is always a cash guarantee;

sureties grantedby entities referred to in art. 6b par. 5 point 2 of the Act of November 9, 2000, the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2014, item 1804 and of 2015, item 978 and 1240).

4. The Ordering Party requests that the Contractor, together with the offer: - - valid as of the day of submitting bids - a declaration on not being subject to exclusion from the proceedings on the form of the Single European Order Documentalso referred to as "EAT" or "a single document", whose editable template is Annex 2 to the ToR. - The EJC pattern specifies the EU Commission Implementing Regulation 2016/7 of 5.1.2016, establishing a standard form for a single European order document ( EU Journal of 6.1.2016, L 3/16).

The information contained in the declaration on the single document form constitutes a preambleconfirmation that the contractor is not subject to exclusion from the proceedings. - A statement on a single document form, made out on pain of nullity in electronic form and bearing a qualified electronic signature, should be submitted via the purchasing platform at http://www.e-ProPublico.pl. The use of the platform is free.

In the case of joint application-for orders by contractors, EAT made in an electronic form and signed by a qualified electronic signature by persons authorized to represent a given contractor, submit each of the contractors jointly applying for the order. This statement is to confirm the lack of grounds for exclusion to the extent specified in the SIWZ. - completed and signed Form of the Offer, in accordance with the model stAnnex No. 1 to SIWZ (or in accordance with its content), - In the scope of confirmation that the offered deliveries meet the specified requirements. Ordering requires:

- Technical description with indication of all technical parameters, meeting the requirements of the Employer specified in the ToR,

- Power of Attorney to represent the contractor (contractors appearing jointly), as long as the offer consists ofplenipotentiary,

- Within 3 days from the date of placing by the Ordering Party on the website information on the opening of bids, referred to in art. 86 par. 5 of the Public Procurement Law, the Contractor is obliged to provide the Employer with a declaration of belonging or non-affiliation to the same capital group referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. With the submission of the statement, the Contractor cane provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure.
