Rental of the next generation sequencer

The subject of the order is the rental of the next generation sequencing (NGS), which enables sequencing by synthesis of genomes, exsom and transcriptomers in so-called short paired readings (paired-end)eads, PE), in accordance with the Ordering Party requirements specified in the Technical Specification for the subject of the order contained in Part IV of the ToR (hereinafter "technical specification").

Short description of the order:

The subject of the order is the next generation sequencing rental, NGS ), enabling sequencing by synthesis of genomes, exsomes and transcriptomers in technso-called short paired-reads (PE), in accordance with the requirements of the Ordering Party specified in the Technical Specification of the subject of the order contained in Part IV of the ToR (hereinafter "technical specification").

The sequencer is to constitute ‡ one integrated system, for which no additional equipment is required.

Detailed description of the subject of the order:

Item forThe speech must be provided together with the operating instructions, controllers, cabling and accompanying software necessary for proper functioning (obtaining full functionality indicated in the technical specification) to the place indicated at the headquarters of the Ordering Party.

The delivered order subject must be brand new, i.e. unused before delivery and in the case of spwithout the use of the correct test necessary to carry out the test.

The equipment on offer can not be provided by the producer for withdrawal from production or sale.

The delivered equipment must come from the official distribution channels of the producer, including the EU market, ensuring specific implementation of rights in gw

Provided equipment requiring power supply from the power grid should be equipped with an appropriate number of power cables allowing for connection to standard power sockets, unless stated otherwise in the technical specification. * The scope of the subject of the order should also include a guarantee and performance of benefits under the guarantee .

As part of the implementationdescribed in this SIWZ subject of the order, Contractor:

- conduct installation and start in the place indicated by the Employer,

- connect the device to the electricity and ICT,

- perform installation, configuration and integration of the systems / software delivered under the order, program individual functions that enable using the device's functionality

Rental requirements:

- Hire the next generation sequencer as described in detail in the Technical Specification of the order included in Part IV of the ToR (hereinafter "technical specification"), for 24 months

- payment for rental on a monthly basis, in equal installments, in PLN

- the subject of the rental remains the property of the Contractor for the whole period of the lease

- insurance of the device

- servicedevice 's designation

The scope of the subject of the order should also include: - a) granting a warranty and providing warranty service (hereinafter referred to as service) -

b) unlimited telephone and email technical consultation during the warranty period.

c) training with service of the order, within the deadline of up to 14 days after its installation and commissioning and after prior arrangement with ZaThe full description of the order can be found in the SIWZ - To confirm the lack of grounds for exclusion from the proceedings referred to in point I 6.1.1 and to confirm compliance with the conditions for participation in the proceedings referred to in point I. 6.2. The Contractor will be required to provide the Employer with the following statements and documents (within the deadlines indicated in this ToR): a) statement WContractors who are preliminary confirmation of compliance with the conditions for participation in the procedure set out in this SIWZ and no grounds for exclusion, submitted on the Single European Order Form (hereinafter referred to as "EAT"), which specifies the Commission Implementing Regulation (EU) 2016/7 of 5.1. 2016 r.

B) excerpt from the relevant register or from central records and business informationif separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 Pzp;

c) certificate of the competent head of the tax office confirming that the Contractor is not in arrears with paying taxes, issued no earlier than 3 months before the deadline for submission of offers or other document confirmingy that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of such receivables together with any interest or fines, in particular he obtained legally exempted, deferred or rescheduled payments, or suspension in full of the decision of the competent authority; the organizational unit of Zakład Ubezpieczeń Sof subsidies or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders or other document confirming that the Contractor has made an agreement with the competent authority regarding repayments these receivables along with possible departuresfines, in particular obtaining the lawful exemption, postponement or spreading overdue payments into installments or suspending the execution of the decision of the competent authority in full,

e) information from the National Criminal Register in the scope specified in art. 24 sec. 1 pts 13, 14 and 21 Pzp and, regarding a conviction for a prison sentence, to the extent specified by the Employer on the basis ofart pond 24 sec. 5 point 5 and 6 of the PPL, issued not earlier than 6 months before the deadline for submitting bids; - f) declaration of the Contractor about the lack of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums; or in the event of such a decision or decision being issued, documentsverifying the payment of these receivables together with any interest or fines or entering into a binding agreement regarding repayment of such debts (with respect to the exclusion referred to in Article 24 paragraph 1 item 15 Pzp); g) a declaration by the Contractor about the lack of a decision against him a preventive measure to prohibit applying for public orders (as regards the premise-the exclusion described in art. 24 sec. 1 point 22 Pzp);

h) Contractor's statement on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2017, item 1785 of late changes). i) declaration of the Contractor about belonging to or lack of belonging to the same capital group.

In confirmation of fulfillment of the condition of participation inin the case of economic or financial situation, the Employer requests from the Contractor documents confirming that the Contractor is insured in the scope of activities related to the subject of the order for the guarantee sum determined by the Ordering Party in point 6.2 points 2) SIWZ. If for justified reasons, the Contractor can not submit documents regarding the financial situationor economic, required by the Employer, may submit another document, which sufficiently confirms compliance with the condition of participation in the procedure described by the Employer. The declaration of belonging or non-affiliation to the same capital group concerns the circumstances referred to in art. 24 sec. 1 point 23 Pzp
