The subject of the order is: Group life insurance for employees and members of the families of employees of Piekarskiego Centrum Medycznego Sp. z o.o. A detailed description of the subject of the order is included in Annexes 1, 2, 5 to the ToR.

The Contractor must prove that he has a license to carry out insurance activityIale I, referred to in the Act of 11.9.2015 on insurance and reinsurance activity (consolidated text, Journal of Laws of 2019, item 381, 730), and if it commenced operations before the entry into force of the Act of on 28.7.1990 about insurance activity (Journal of Laws No. 59, item 344, as amended), a certificate from the Minister of Finance on having a consent to perform insurance activitiesabove.

1. The offer must be secured by a bid bond in the amount of PLN 60, 000.00 (in words: sixty thousand zlotys 00/100).

2. Submission of offers takes place only through the shopping platform https: //pcm-piekary.ezamawiaćacy.pl

3. Information on the processing of personal data is contained in the ToR in point 20. The administrator of personal data is Piekarskie Centrum Medyczne Sp. z o.o.

4. For orderingContractors who are not subject to exclusion pursuant to art. 24 sec. 1 item 12 - 23 of the Public Procurement Law - 5. In addition, the Procurer will exclude the Contractor pursuant to art. 24 sec. 5 points 2 and 4 of the Public Procurement Law Act (subject to the deadlines specified in Article 24 (7) of the Public Procurement Law).

6. Together with the offer, the Contractor must submit in the form of a Single European Order Documentsubscription signed by a qualified electronic signature valid as of the day of submitting bids constituting preliminary confirmation that the Contractor is not subject to exclusion from the proceedings and meets the conditions for participation in the proceedings. The ordering party shall call the Contractor whose bid has been best assessed before submitting the order, to submit in a designated, not shorter than 10 days, terminie of the following declarations and documents valid as at the date of submission of the declarations and documents, subject to art. 26 par. 6 Public Procurement Laws:

7.1. permission to carry out insurance activity in accordance with the provisions of the Act of 11.9.2015 on insurance and reinsurance activities (consolidated text, Journal of Laws of 2019, item 381, 730), ) in the scope of insurance covered by the order or in the case of operatingon the basis of a permit other than the subject-matter, another document entitling to conduct insurance activity in the territory of the Republic of Poland in the scope of insurance covered by the order.

7.2. information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting offers,

7.3 oproviding the contractor with the lack of a final court judgment or final administrative decision on the payment of taxes, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming payment of such receivables, including any interest or fines or conclusion binding antigenon the repayment of these debts,

7.4 the contractor`s statement about the lack of a decision against him as a preventive measure for the prohibition of applying for public orders.

8. Contractor, whose offer will be assessed the highest at the request of the Ordering Party in accordance with art. 26 par. 1 of the PZP Act, the following documents concerning the subject of the order:

8.1. General insurance conditions of the applicationimportant risks,

8.2. General conditions for group supplementary insurance,

8.3. Table for the assessment of the percentage of permanent health detriment,

8.4. Catalog of surgical operations,

8.5. Conditions for individual insurance continuation

9. Reporting to the proceedings requires logging the Contractor to the System on the subdomain https://pcm-piekary.ezamawiajacy.pl, or https://oneplace.marketplanet.pl. Registration of ContractorThe maximum duration is 2 business days. Therefore, the Contracting Authority recommends the Contractors to take into account the time necessary for registration in the process of submitting the Offer in electronic form. The order will be implemented for a period of 48 months from the date of commencement of insurance services, but not earlier than from 1/10/2019. In the case of a contract from 1/10/2019, the contract will last until 30.9.2023r.

Contractor within 3 days from the date of posting on the website information referred to in art. 86 par. 5 of the Public Procurement Law, will provide the Employer with a declaration.
