Provision of group life insurance services for employees and their family members of the Warsaw Medical University and medical units WUM

Reference Number: AEZ / S-142/2018

1. The subject matter of this order is the provision of group employee insurance services and members of their families:

1. The subject matter of this order is the provision of servicesinsurance of employees and their family members:

a) of the Warsaw Medical University with headquarters at ul. Żwirki i Wigury 61, 02-091 Warsaw,

b) University Clinical Center of the Medical University of Warsaw with headquarters at ul. Banacha 1a, 02-097 Warsaw,

c) Clinical Hospital Fr.. Anna Mazowiecka in Warsaw with headquarters at ul. Karowej 2, 00-315 Warszawa,

2. The scope and minimum conditions for the provision of group life insurance services for employees of the Employer and their families, as well as optional terms, constituting the criterion for the evaluation of offers, in accordance with the provisions of Chapter XIV of the ToR, are specified in the Order description, Annex 2 to the ToR.

3. Given that the Employer can not guarantee that all employeesy and their spouses and adult children join the life insurance, for the purpose of these proceedings, the Employer provided for a maximum group of people who will be able to take out insurance (6 896 people), corresponding to the number of people employed in the Employer's units, and also based on the employment structure people currently using group insurancefor life (4 638 people), he predicted 5 insurance options that will apply depending on the individual decision of the insured and additional insurance that will apply to each of the above-mentioned variants as supplementary. The Employers' employment structure is defined in Annex 1 to the Description of the Subject of the Order.

4. The ordering party requires, under pain of rejection of the offer, aso that the monthly premium of the cheapest variant does not exceed PLN 52.50, the most expensive variant does not exceed PLN 100, and the difference in premiums between successive options was not less than 6% of the most expensive premium offered. In addition, the value of the additional insurance premium may not exceed 30% of the most expensive premium from variant

5.In the light of the above assumptions, the amount of funds allocated to finance the order within 24 months from the date of accession of the insured to the subject of the contract takes into account the highest possible scope of the order, constituting the right of option within the meaning of art. 34 par. 5 Public Procurement Laws

6. The Contracting Authority reserves the right not to use the subject of the contract in terms of value or coverage of povertywith a smaller number of people than the planned 6, 896 people. In this case, the Contractor will not be entitled to any claim due to the Ordering Party not using the subject of the order in the full quantitative or valuable range, including the selection of the variant of insurance (see -2 clauses 9-10 of the Contract Model). In accordance with the provisions of Chapter VII of the Terms of Referenceamateurs, based on art. 67 sec. 6 of the Act, provided for the provision of an additional order consisting in the repeated services covered by the subject of the contract for a subsequent period, up to 24 months from the date of completion of the contract originally concluded, with the proviso that the total amount of these services will not exceed the amount specified in the original contract. Based on art. 67 sec. 1 point 6 of the Act, the Procurer prit will provide for the ordering of an additional, consisting of the repeated services, in accordance with the information contained in Chapter III, para. 7 of the SIWZ for the next period, up to 24 months from the date of completion of the contract originally concluded, with the proviso that the amount of these services will not exceed the amount specified in the original contract.

The Contracting Authority will consider the condition to be fulfilled if the Contractor demonstrates that he has a permitnot to perform insurance activities referred to in the Act of 11.9.2015 on insurance and reinsurance activities (Journal of Laws of 2018, item 999), and in the event that the Contractor commenced operations prior to the entry into force of the Act of 28.7.1990 on insurance activity (Journal of Laws No. 59, item 344, as amended), a certificate from the Minister of Finance on having permission to exerciseinsurance activity.

The Ordering Party does not specify any specific conditions in this respect - I). The present proceedings are conducted: -a) under an open tender procedure, in accordance with art. 39 et seq. of the Act, with the value of orders above the amount specified in the implementing regulations issued on the basis of art. 11 para. 8 of the Act. The contracting party also explains that in the post-operatedreferred to in Article 24a (a) 1 of the Act, will first evaluate the offers, and then examine whether the Contractor whose offer has been evaluated as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings, - b) using the e-ordering platform of the Open Market Elektroniczny SA Marketplanet, Domaniewska 49, 02-672 Warsaw, POLAND, hereinafter referred to as "Platform ", ensuring that this procedure is carried out in a manner consistent with the provisions of Chapter 2a of the Ordering Party's Communication with Contractors and described in detail in this Terms of Essential Terms of Orders, in particular Sections XVIII and XIX
