1. The subject of the order is a group life insurance for employees and members of the employees' families Gornośląskie Centrum Zdrowia DI will help them St. John Paul II Independent Public Clinical Hospital No. 6 of the Medical University of Silesia in Katowice.

1. The subject of the order is a group life insurance pRacownikow and family members of employees Gornośląskie Centrum Zdrowia Dziecka St. John Paul II Independent Public Clinical Hospital No. 6 of the Medical University of Silesia in Katowice

2. The Employer does not guarantee that all employed employees and family members of the Employer's employees will join the group life insurance.

3. Contracts may occur during the term of the contractin the number of insured persons.

4. If an employee or employee's family member resigns from insurance, the Employer is not obliged to pay for them an insurance premium.

1. meeting the conditions set out in Article 22 par.1b point 1 in the scope of possessing competences or entitlements to bring about a specific professional activity, i.e. having a permit to perform activitiesinsurance referred to in the Act of 11.09.2015 on insurance and reinsurance activities (unified text Journal of Laws of 2017 item 1170), and in the case when they started operating before the entry into force of the Act of on 28.7.1990 about insurance activity (Journal of Laws No. 59, item 344 as amended), the Minister of Finance's certificate about having permission to perform actsinsurance 2. for which there are no conditions resulting in exclusion from the proceedings set out in: - a) art. 24 sec. 1 point 12-22 of the Act;

b) art. 24 sec. 5 point 2 of the Act - The Contractors against whom on the basis of their mid-circulation equipment the Employer would be excluded from the proceedings showed that as a result of gross negligence, they performed the contract improperly. Irritating carelessness lectureIn particular, the term is understood as: - unjustifiable exceeding of the deadlines specified in the ToR, GTC and legal provisions for liquidation of damages, - 2) conducting the acquisition in a manner inconsistent with the conditions specified in the ToR, - 3) non-compliance with the document work clause in force at the ToR

The Purchaser will inform the Contractor in writing of the exclusion providing its factual justification and rightswith an indication of the evidence on which the decision was made.

c) Art. 24 sec. 5 point 8 of the Act

3. Self-cleaning measures

Contractor who is subject to exclusion pursuant to art. 24 sec. 1 item 13, 14, 16-20 and art. 24 sec. 5 points 2 and 8 of the Act may on the basis of art. 24 sec. 8 of the Act, provide evidence that the measures taken by the means are sufficient to demonstrate its reliability, in particular, to prove the remedy of an offense managed by an offense or a tax offense, financial compensation for damage suffered or remedied, an exhaustive explanation of the facts and cooperation with law enforcement agencies and taking specific technical, organizational and human resources measures that are appropriate to prevent further offenses or criminal offensesTax or improper conduct of the Contractor.

The application of remedy measures will not take place in relation to the Contractors being a collective entity, the Contractor against whom the court ruled by the binding verdict against applying for the order and for which the period of validity of the prohibition does not expire.

The Contractor is not subject to exclusioniu, if the Employer, considering the weight and special circumstances of the Contractor's action, deems sufficient evidence presented by him. If the evidence provided is not taken into consideration, the Ordering Party may exclude the Contractor. A) the Contractor has a third party liability insurance in the amount of 1, 000, 000.00 PLN;

1. Wadium - the bid should be secured with a deposit of 30.000 PLN. The bid security should be submitted in the form indicated in the ToR before the deadline for submission of tenders. Repetition of services similar to the basic order - The Contracting Authority provides for the possibility to conclude during the contract term the order consisting in the re-establishment of services similar to the basic order constituting 20% of the basic order. The order will be granted for services consistent with the objectm basic order in the mode of single-source procurement, after fulfilling the conditions of art. 67 point 1 point 6 of the Act

3. Amendments to the contract - 1. The orderer allows the possibility of making changes to the concluded insurance contract. Admissible changes concern:

1) updating the subject of insurance and insurance sums. In the event of the item being updated and the amount insured, the Contractor shall issue confirmationschanges introduced with the addition of additional premiums or information about its possible return. The amount of the additional premium or the amount of its reimbursement will be calculated using the rate applied in the tender in the pro rata temporis system - 2) the date of order completion, including early termination of the contract due to circumstances that the Awarding Entity could not anticipateby ordering or renewing the contract until the conclusion of the contract for awarding a new contract;

3) the implementation of additional and necessary services from the current Contractor after fulfilling all the deferral specified in art. 144 para. 1 point 2 a-c of the Act;

4) if the conditions specified in art. 144 para. 1 point 3 a-b of the Act;

5) change of the Contractor to whom he ordersgranting the order and replacing it with a new Contractor to fill one of the premises specified in art. 144 section 1 point 4 a-c of the Act;

6) changes, regardless of their value, which are not significant within the meaning of art. 144 para. 1e of the Act;

7) changes, the total value of which is less than the amounts specified in the regulations issued on the basis of art. 11ust. 8 and is less than 10% of the order value approxoriginally remarked in the contract

8), the amount of remuneration due to the Contractor will change if: a) the rate of tax on goods and services;

b) the amount of the minimum remuneration for work or the minimum hourly rate, determined on the basis of the provisions of the Act of 10.10.2002 minimum pay for work, c) rules of being subject to social insurance or insuranceor change in the amount of social security or health insurance;

The change from point 8) may be made at the request of the Contractor, which will show the relevant effect on the cost of the order in a proper manner. Contractors operating in the form of a mutual insurance company - if the Contractor is acting in the form of mutual insurance, in the event of the contract being awarded to himthe contract will not be included in the recipe of reciprocity.
