The subject of the order is the supply of printing equipment, consisting of two tasks, tj.:

Main Site: Area of the city of Wrocław

1. Task No. 1 delivery of 51 pieces of network laser devices by printings.

2. The Contractor is obliged to provide identical, original and originating equipment from one producer from his legal distribution channel. The Contractor is obliged to pay a bid bond in the following amount: - a) for the Task No. 1 - PLN 1, 200.00 (in words: one thousand two hundred zlotys).

B) for the Task No. 2 - PLN 3, 000.00 (in words: three thousand zlotys) ).

2. The ordering party requires in this afterconfirm confirmation by Contractors that there is no exclusion against them under Art. 24 sec. 1 and par. 5 points 1 and 8 of the Public Procurement Law. The Employer also provides for the possibility of exclusion from the Contractor's conduct:

a) who in a manner of culpable seriously violated professional duties, which undermines his integrity, in particular when the Contractor as a result of intentionalact or gross negligence did not perform or improperly performed the order, which the Purchaser is able to demonstrate by means of appropriate evidence (Article 24 paragraph 5 point 2 of the Public Procurement Law);

b) who, for reasons attributable to him, did not perform improperly performed, to a significant degree, an earlier public procurement contract or concession contract with the ordering party, mentioned in art. 3 par. 1 point 1-4 of the Public Procurement Law, which led to the termination of the contract or awarding damages (Article 24 (5) (4) of the Public Procurement Law).

3. In confirmation of the lack of grounds for exclusion, the Employer will require declarations and documents specified in - 5 points 1) -6) and 10) of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Employer may request from thein the proceedings for the award of the contract (Journal of Laws of 2016, item 1126).

4. The Contractor is obliged to submit a Single European Order Document. The information contained in the EED is a preliminary confirmation that the Contractor meets the conditions for participation in the procedure and is not subject to exclusion from the procedure.
