1. The subject of the order is the supply of reagents and equipment along with the lease of equipment for the needs of the laboratories of the Central Clinical Hospital of the Medical University of Lodz at ul. Pomorska

1. The subject of the order is the purchase and supply of reagents and equipment along with the lease of equipment for the needsy laboratories of the Central Clinical Hospital of the Medical University of Lodz at ul. Pomorska 251

2. The detailed description and scope of the order is set out in Annexes 2 and 3 to the Terms of Reference (SIWZ).

Documents confirming that the contractor is insured against civil liability in respect of activities related to the subject of the order for catfishwarranty determined by the ordering party as an attachment;

I. For the entire order, the Contracting Authority requires a deposit of PLN 17 123, 00 (in words: seventeen thousand one hundred and twenty-three zlotys zero grosz) in accordance with art. 45 of the Public Procurement Law. The bid security should be submitted before the deadline for submission of offers. The value of the bid bond for the entire offer of a given Contractor is the sum of wades per offerpackages. The value of the bid bond for individual bundles is provided in SIWZ - chapter X.2

II. Pursuant to the Regulation of the Prime Minister of 27.7.2016 on the types of documents that the contracting authority may request from the contractor in the proceedings for the award of the contract, and the forms in which these documents may be submitted (Journal of Laws of 2016, item 1126 ), to confirm the absence of grounds for exclusionand contractors from participation in the proceedings for the award of this order, the Employer will demand from the Contractor the highest rated of the following documents:

1. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids or applications for admission to participate in the procedure,

2.-Certificates from the competent head of the tax office confirming that the contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submitting bids or applications for admission to participate, or other document confirming that the contractor has concluded an agreement with the competent tax authority repayment of these receivables, inclinterest, or fines in particular, obtained legally required exemption, postponement or spreading into installments of overdue payments or suspension in full of the implementation of the decision of the competent authority;

3. The certificate of the relevant local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractorand it is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids or applications for admission to participate, or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of these receivables with any interest or fines, in particular obtainedrocks provided for by law, release, postponement or installment of overdue payments into installments or suspension in full of the implementation of the decision of the competent authority;

4. A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points1 of the Act - III. In order to confirm that the offered deliveries meet the requirements specified by the Ordering Party, the highest rated contractor will be obliged, upon the request of the Employer, to submit the following documents:

1. Declaration (Annex No. 7) on the compliance of the products offered
