1. The subject of the order is the supply of reagents and control materials along with the lease of analyzers for SPWSZ in Szczecin. The subject of the orders dividedest for 11 tasks (parts).

A detailed description of the subject of the order is attached as Appendix 1 to the ToR and Annex No. 1A-1 to the Terms of Reference for task # 1. Task No. 1 consists of 7 items

1 . Each contractor is obliged to:

1) submit a statement before the expiry of the deadline for submission of offers, in order to satisfy. the conditions of participation in the proceedings and the lack of premises for exclusion of the Contractor. Declaration. in accordance with art. 25a para. 2 of the Public Procurement Law, the Contractor submits in the form of a Uniform European Order Document (EIN), according to the state formula. Annex No. 3 to the SETC. Attention! Declaration. EAT is submitted only in electronic form, provided-by qualified electronic signature.

2) attach to the offer (prepared according to the template in the state of appendix No. 2, 2A to SIWZ):

a) required boundary conditions for the task. No. 1-11, according to the state formula. Annex No. 1A to SIWZ,

b) parameters subject to qualitative assessment for the task. No. 1-11, according to the state formula. Annex No. 1B to the Terms of Reference, - c) obligations of third parties, according to the state formula. Annex No. 5 to the Terms of Reference in the case of appointmentby the contractor for the resources of other entities in order to demonstrate the fulfillment of conditions for participation in the proceedings, d) appropriate power of attorney or documents (eg extracts from the register) showing the authority of the person signing the offer to represent the Contractor (if applicable), - e) documents describing the products offered by the contractor (catalog pages or folders, information leaflets, etc.) in Polish-for the offered products (concerns tasks no. 4 and 8),

f) instructions / operating instructions for the leased analyzer / leased analyzers (applies to tasks No. 1-3, 5 and 6).

2. The Contractor is subject to exclusion in case any of the conditions specified in art. 24 sec. 1 item 12-23 of the PPL or art. 24 sec. 5 points 1 and 8 Pzp.

3. In order to make a monster satisfy. conditions of participation in the proceedings and lack of underthe contractor's exclusion from participation in the tender, ordering prior to the order being awarded, will call the contractor whose offer has been rated the most, to submit within the prescribed, not shorter than 10 days, the current date on the day of submitting the following declaration. and documents:

1) information from the NQF in the scope specified in art. 24 sec. 1 point 13, 14 and 21 PPL Acts,

2) certification of the competent headUS confirm that the contractor is not in arrears with paying taxes,

3) a certificate of the relevant ZUS or KRUS organizational unit or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, - 4) a copy from the relevant register or from central records and business information, if separate regulations require an entry in the registeror records, for the purpose of no grounds for exclusion under Art. 24 sec. 5 point 1 of the PPL Act,

5) Decree The Contractor on the lack of issuing a final judgment of the court against him or a final administrative decision on being in arrears with the payment of taxes, fees or social security or health insurance premiums,

6) declare Contractor on the lack of a decision against him as a preventive measure of the prohibition-applying for a public order,

7) declare Contractors about not paying taxes and local fees,

8) declare Contractor about belonging or not belonging to the same capital group, referred to in art. 24 sec. 1 point 23 of the Act,

9) in order to demonstrate the fulfillment of the condition of participation in proceedings regarding the possession of technical or professional capacity, referred to in Chapterbut V point 1 point 2 letter c) of the ToR:

a) list of persons, directed by the contractor to implement the public order, which model state. Annex No. 6 to the Terms of Reference, together with information on their professional qualifications, entitlements, experience and education necessary to perform the public order, as well as the scope of their activities and information on the basis of their useand people.

4. In order to confirm that the offered deliveries meet the requirements specified by the ordering party, the Ordering Party prior to the order being awarded, will call the contractor whose offer has been assessed at the most, to submit in the shortened period, valid as of the day of submitting the declaration. or documents specified in Chapter VI, point 11
