The subject of the order is successive delivery:

1. The subject of the order is the successive supply of reagents for the determination of glycated hemoglobin with the lease of equipment for testing bythe Laboratory Diagnostic Order of the Employer. The details concerning the subject of the order are described in Appendix 2A (constituting the assortment and price form at the same time).

2. The conditions for the execution of the order have also been specified in the contract's designs. The Employer does not set the conditions for participation in the procedure. The Employer does not set conditions for participation in the procedure.they must submit together with the offer (according to the template, which constitutes the offer form - print of the offer constituting Annex 2 to the SETC together with Appendix 2A in the assortment and price form):

1.1. Present on the day of submitting bids a statement in the form of a single European order document (hereinafter: EAT) in the scope indicated in SIWZ in Annex No. 1 to the SETC (pattern of EAT) and in the instructions filled inand EAT, which is described in Chapter XXIII of the ToR and in Chapter XXIV of the ToR - Instruction on the requirement to submit an electronic version of EAT. The information contained in the EED is a preliminary confirmation that the Contractor is not subject to exclusion. EAT should be prepared in accordance with the standard form provided for in the European Commission Implementing Regulation issued on the basis of art. 59paragraph. 2 of Directive 2014/24 / EU. The pattern of EAT is Annex 1 to the ToR. Contractors are required to submit a Single European Order Document (EAT) in electronic form with a qualified electronic signature. In this public procurement procedure, the offer, as well as the power of attorney and the obligation of the third party, are submitted in writing, and EATin electronic form.

To complete the order of art. 25a para. It is sufficient for the EAT to be transferred to the Ordering Party via the means of electronic mail communication to the address indicated in Chapter VIII of the ToR, before the deadline for submitting bids
