Deliveries of reagents for the determination of acid-base balance and carbon monoxide-hemoglobin

Reference Number: WSzSL / DZ-66/18. The subject of orders is the successive supply of reagents for the determination of acid-base balance (RKZ) and carbon monoxide (COHb) to perform tests by the Laboratory Diagnostic Laboratory of the Employer togetherwith a rental analyzer and a computer set with software and a scanner. The details concerning the subject of the order are described in Appendix 2A (constituting the assortment and price form at the same time).

1. The subject of the order is the successive supply of reagents for the performance of acid-base balance (RKZ) and carbon monoxide (COHb) determinations to perform the testby the Laboratory Diagnostics Department with the hire of an analyzer and a computer set with software and a scanner. The details concerning the subject of the order are described in Appendix 2A (constituting the assortment and price form at the same time).

2. The ordering party does not allow partial offers.

3. The conditions for the execution of orders set out fromthey have also been included in the draft agreements in Chapter XVII siwz.

1. Contractors must submit together with the offer (according to the formula, which is the bid form - print offer constituting Annex 2 to SIWZ together with Appendix 2A in the assortment and price form):

1.1. Current as of the day of submitting offers-a statement in the form of a single European order document (hereinafter: EAT) in the scope indicated in SIWZ in Appendix No. 1 to SIWZ (pattern of EAT) and in the instructions for completing EAT, which is described in Chapter XXIII SIWZ and in Chapter XXIV siwz -Instruction regarding the requirement of submission electronic version of EAT. The information contained in the EAT is a preliminary confirmation that the Contractor is not pdistances are excluded. EAT should be prepared in accordance with the standard form provided for in the European Commission Implementing Regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU. The EAT formula is Annex 1 to SIWZ. Contractors are required to submit a Single European Order Document (EAT) in electronic form with a qualified electronic signatureronicznym. In the present proceedings for the award of a public order, the offer, as well as the power of attorney and the obligation of the third party, are submitted in writing and the E-mail in electronic form.

To fulfill the order of art. 25a para. 2 uPzp it is sufficient that the EAT is handed over to the Ordering Party by means of electronic mail communication to the address indicatedin the Chapter VIII siwz, before the deadline for submission of tenders.

In the case of joint application for contracting by the Contractors, the declaration (EAT) is submitted by each of the Contractors jointly applying for the order - These statements are to confirm the lack of grounds for exclusion to the extent that each of Contractors show no grounds for exclusion.

1.3. In order to evaluate offers in the scope of "functional and operational parameters" of offered analyzers, the Contractor is obliged to submit documents with the offer regarding the offered item, ie: issued / published by the manufacturer of the offered analyzer - information leaflets, folders or parts folderow, technical cards, catalogs or parts of catalogs, etc., in which the characteristics, parameters, properties, etc. of the offered item are described / indicated in the scope of parameters subject to assessment by the Employer and offered by the Contractor. In the event that the Contractor fails to submit the above-mentioned documents along with the offer, despite the fact that the Contractor will indicate and declare in Annex 2A the offer of a given itemThe ordering party will not reject the offer, but will consider that the offered assortment does not have the parameter / parameters evaluated and will award 0 partial points in this respect.

2. Contractors, who are not subject to exclusion pursuant to art. 24 sec. 1 item 12-23 of the Public Procurement Law and art. 24 sec. 5 point 1 uPzp.
