Before the order is awarded, the contracting authority will call the contractor whose offer has been rated the most, to submit in a period not shorter than 10 days valid as at the day of submitting declarations or documents: 1) excerpt from the relevant register or from central records and business information, ifdrastic provisions require an entry in the register or records, in order to confirm the absence of grounds for exclusion under art. 24 sec. 5 points 1 of the Public Procurement Law; 2) information from the NQF within the scope specified in art. 24 sec. 1 item 13, 14 and 21 of the Act, 3) the contractor's statement that the court has not been the subject of a final court decision or a final administrative decision on late payment of taxes, fees orsocial insurance or health insurance contributions 4) declaration of the contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders. Detailed information on documents is provided in Chapter V of the TOR. The Contractor shall submit a statement valid at the day of submitting bids in the scope indicated by the ordering party in the contract notice or in the relevant specificationh order conditions in the form of EAT in electronic form with a qualified electronic signature. The electronic communication means used for depositing EAT by the contractor is e-mail. The information contained in the EED is a preliminary confirmation that the contractor is not subject to exclusion. The ordering party must attach a price form filled out by the Contractor to the offer-for the package offered. The Contractor who refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that they refer to their resources, the condition of participation in the proceedings also submits uniform documents regarding these entities on the EAT form, which must take the form of an electronic document signed with a qualified pelectronic deduction by each of them to the extent that each of the Contractors demonstrates fulfillment of the conditions for participation in the proceedings and no grounds for exclusion. An analogous requirement applies to EAT submitted in the case of joint application by contractors. These documents confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent to which everyonethe contractors demonstrate compliance with the conditions for participation in the proceedings and no grounds for exclusion
