Supply of analyzers for the needs of the Pomeranian University in Słupsk

The subject of the order is the supply of analyzers for the needs of the Academy Pomorska in Słupsk.

The subject of the order consists of the following elements:

- analyzer for testing critical parameters 1 item,

- analyzer for biochemical tests 1 item,

- immunochemical analyzer 1 item

The Employer does not set a detailed condition in this respect. Contractors who are not subject to exclusion from the public procurement procedure may apply for the award of the contractego in the circumstances referred to in art. 24 paragraph 1 point 12 23 and par. 5 item 1 of the PPL and meet the conditions for participation in the procedure. Before awarding the contract, the Awarding Entity will call the Contractor whose tender has been rated the highest to submit within 10 days valid on the day of submitting the statements or documents:

1) an excerpt from the relevant register or from the central records andinformation on economic activity, if separate provisions require an entry in the register or a register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;

2) information from the KRK to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act;

3) the Contractor`s statement that he has not issued a final court judgment or final administrative decisionj on arrears in the payment of taxes, fees or social security or health insurance contributions;

4) the Contractor`s statement of the lack of a decision against him as a preventive measure forbidding public procurement;

The offer must contain the following statements and documents:

a) completed offer form prepared on the basis of the template constituting Annex No. 1 / 1.1 to the Terms of Reference;

b) EAT declaration, referred to in Chapter V, para. 1 3 of this ToR, Annex 2 to the ToR;

c) table of technical parameters of the offered subject of the order constituting Annex 3 to the ToR withdetailed description of the parameters offered;

d) if the third party`s commitment is signed by a qualified electronic signature;

e) the original guarantee / surety, if the Contractor lodges a deposit other than in cash;

f) if the power of attorney referred to in Chapter . VI section 1 point 4 SIZW bearing a qualified electronic signature;

g) documentdeclaring the appointment by the Contractors jointly applying for the contract, a representative to represent them in the contract award procedure or to represent them in the procedure and conclusion of the public procurement contract (applies only to Contractors jointly applying for the contract) the document should be presented in the form provided for in Chapter . VI section 1 point 4 SIWZ for a power of attorney.

Offer with attachments, statement referred to in art. 25a, including a single document (EAT), shall be made, under pain of nullity, in an electronic form and shall have a qualified electronic signature. Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent thatrefers to their resources, the conditions for participation in the procedure also submits uniform documents regarding these entities on the EAT form, which must be in the form of an electronic document, signed by a qualified electronic signature by each of them to the extent that each of the Contractors shows no grounds for exclusion or compliance with the conditions participation in the proceedings. Analogous requirement dapplies to EAT submitted when Contractors jointly apply for the order.
