ZP / 76/2018 Delivery of an integrated confocal microscopy system.

Subject orders are delivered in 8 packages of an integrated confocal microscopy system for automatic visual analysis of cells, tissues, spheroids and the bodymodel ovens along with the automation system for preparing, storing and administering samples along with room adaptation for the Laboratory Diagnostics Medical Department. The subject of the order consists of the following packages, described in detail in Annex 3 to this SIWZ:

Contractors who are not subject to exclusion from the public procurement procedure in the circumstances referred to in Art. 24 sec. 1 point 12-23 and item 5 point 1) PPL and meet the conditions for participation in the proceedings. Before ordering, the ordering party will call the contractor whose offer has been rated the most, to submit in terminie not shorter than 10 days valid as at the date of submitting statements or documents: 1) excerpt from the relevant register or from central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion pursuant to art. 24 sec. 5 points 1 of the Public Procurement Law; 2) information from the NQF within the scope specified in art. 24 sec. 1-points 13, 14 and 21 of the Act, 3) statements of the contractor on the lack of issuing a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance fees 4) declaring the contractor about the lack of a ruling against him as a precautionary measure prohibition to apply for public orders. Detailed information fromthe documents are contained in chapter V of the SIWZ. The Contractor shall submit a statement, valid as of the day of submitting bids, to the extent indicated by the ordering party in the contract notice or in the specification of relevant order conditions in the form of an EAT in electronic form bearing a qualified electronic signature. The e-mail is the means of electronic communication used for depositing EAT by the contractorical. The information contained in the EED is a preliminary confirmation that the contractor is not subject to exclusion.

The contracting party must include a form and price form filled in by the Contractor, a completed statement of parameters - Contractor who relies on the resources of other entities in order to demonstrate the lack of their existence grounds for exclusion and compliance, in so far as it isthey are obliged to participate in the proceedings, they also submit uniform documents regarding these subjects on the EAT form, which must be in the form of an electronic document signed with a qualified electronic signature by each of them to the extent that each of the Contractors demonstrates fulfillment of the conditions for participation in the proceedings and no grounds for exclusion.
