The subject of the contract is the establishment of hospital kitchens - delivery of technology according to the elaborated project for the needs of the patient`s kitchen of the Teaching Hospital of J. A. Reiman Prešov. The subject of the contract is divided into 2 separate complete parts. The inseparable part of the contract is transport to the agreed place of delivery, unloading at the place of delivery, complarranging and putting the object of the order into operation, training the person about the correct use of the subject of the order with the issuance of a training certificate containing a name list of the trained personnel and providing service services during the guarantee period. Applicant will ensure a permanent presence of at least 1 professionally competent person for a period of 1 month, which will be present at the place of delivery and installation of the subject of the contract afterthe time available is always available every working day from 07.00 hrs. to 3 pm for the purpose of professional supervision and to assist the present public contract staff in the operation and use of the subject matter of the contract by all his / her sites. nand parcel of register C, parcel number: 4869/1 built in the Prešov district

The subject of the contract is the establishment of hospital kitchen - delivery of gastro technology without cooling according to the elaborated project for the needs of the patient kitchen of the Faculty Hospital and Health Center of J. A. Reiman Prešov. The inseparable part of the contract is transport to the agreed place of delivery, unloading at the place of delivery, completionand putting the object of the order into operation, training the person about the correct use of the subject matter of the order with the issuance of a training certificate containing a name list of the trained personnel and providing service services during the guarantee period. Applicant shall ensure the permanent presence of at least 1 professionally competent person for a period of 1 month, which shall be present at the place of delivery and installation of the subject of the contract after the specified date.both are available every day of the day from 07.00 hrs. to 3 pm for the purpose of professional supervision and to assist the present person of the contracting authority in the operation and use of the subject matter of the contract in all its aspects.also the issuing or securing of the issuance of the certificates or confirmation of the execution of these legal regulations of the required checks and measurements. In this respect, the bidder is particularly committed to supplying so-called documentation. Good Manufacturing Practice, ie Hazard Analysis and Hazard Analysis and Critical Control Points (HACCP).the subject matter of the order and its proper use of the necessary software driver and license to them, as well as the provision of these software drivers and the granting of all licenses to them to the extent necessary for proper, undisturbed and unlimited use of the subject matter including its software components and accessories

The provision of written documents is part of the ordernecessary to acquire ownership right to the subject matter of the contract, to provide user documentation for the subject matter of the contract. All written documents must be written in the Slovak language.

Detailed definition of the subject of the contract is given in Part B Description of the subject of the petition. 1 zakonaon public procurement. Applicant demonstrates compliance with the conditions of participation in personal status pursuant to the provisions of - 32 para. 2 of the Public Procurement Act by submitting documents:

a) pism. (a) evidenced by a statement of criminal records not older than 3 months; (b) substantiated by an attestation by the health insurance company and the Social Insurance Agency not older than 3 months; (c) substantiated by confirmationof the relevant tax office not more than 3 months old,

d) pism. (d) evidenced by an acknowledgment of the relevant keg not older than 3 months; (e) documented proof of entitlement to supply the goods, carry out construction work or provide a service which corresponds to the subject of the contract; (f) by an affidavit.

Applicant must also satisfy the condition of the provision of Act no. 91/2016 Coll. about tresof the legal entities and on the amendment and supplementation of some laws, which came into force on 1.7.2016. As a result of this legislation, economic entities with a status of a legal entity were set up to prove their personal status pursuant to Article 32 (1). 1 pism. (a) public procurement prohibition to submit a copy of the criminal record for a legal person. Candidates / Interestc. entered in the List of Economic Entities maintained by the Public Procurement Office, the documents required to prove compliance with the conditions of participation pursuant to - 32 para. 1 pism. a) to f) no. 343/2015 Coll. on public procurement by submitting information on its enrollment in the list of economic operators, eventually confirming its enrollment in the list of economic entities pursuant to - 152public procurement law.

3. If the applicant, the place of business or habitual residence outside the territory of the Slovak Republic and the statutes of his / her place, place of business or habitual residence do not issue any of the documents listed in the provision of - 32 para. 2 of the Public Procurement Act, or does not issue equivalent documents, may be replaced by a solemn declaration in accordance with the rules in force in its Statebusiness, place of business or usual residence.

4. If the applicant`s residence, place of business or habitual residence outside the territory of the Slovak Republic is not governed by an institute of honorary pronouncement, it may replace it by a declaration made before a barrel, a regular organ, a notary, another professional institute or a business institute according to the regulations in force in the state of the institution, place of business or Usual

Public contracting authority pursuant to - 152 para. 3 of the Public Procurement Act recognizes an equivalent record or a certificate of registration issued by the competent authority of another Member State to which the applicant proves that the conditions of participation in the public procurement have been met. The contracting authority must also accept other equivalent documents submitted by the applicant. A bidder that is a group of vendorsdemonstrates the fulfillment of the conditions of participation in the public procurement of personal status for each member of the group separately. The right to supply goods, to carry out construction work or to provide a service shall be demonstrated by the group member only in relation to that part of the object of the contract to be provided. Applicant may, in accordance with - 39 of the Public Procurement Act, provisionally replacecompliance with the conditions for participation determined by the contracting authorities through the Single European Document (JED). The submission / sending of original scans or a certified copy of all documents required under this Participation Condition is required.to ensure that their content is permanently captured by the Eranet system available at https://fnsppresov.eranet.sk (hereafter Eranet).
