The subject of the contract is to provide service application support for IIS MO SR, IIS MO SR development and licensing and the IIS MO SR support license.

The subject of the contract is the provision of service application support of IIS MoS SR, development of IIS MO SR and licensing and licensing of IIS MO SR. Conditions of participation of applicants in public procurement related to personal status pursuant to - 32 of the Public Procurement Act

1.1. Information and formalities necessary to assess compliance afterterm of service: The entrant must meet the conditions of - 32 par. 1 of the Public Procurement Act

1.1.1. The applicant shall prove that he / she fulfills the conditions of his / her personal status under - 32 par. 2, resp. ods. 4, 5 by submitting original documents or certified true copies of the documents, pursuant to Section 152 of the Public Procurement Act

1.1.2. Entrepreneur entered on the list of economic entitiesaccording to the regulations effective until April 17, 2016, the fulfillment of the conditions of part time pursuant to - 32 par. a) if, in its organizational structure, the member / members of the supervisory body and / or the procurator / procurator show the following:

1.1.2.1. according to - 32 par. (a) by submitting an extract from the criminal record not exceeding three months; or

1.1.2.2. pursuant to - 152 of the Act on Public Procurement (after the change of udincluding in the list of economic entities and adding this information to the list of economic entities pursuant to Section 156 of the Public Procurement Act)

1.1.3. Agents entered on the list of economic entities pursuant to the regulations effective until 17 April 2016 fulfillment of the conditions of participation in accordance with Article 32 of the Decree. b) letter. c) show the following:

1.1.3.1. according to - 32 par. b) letter. c) by submitting a health certificate aftersocial security and social insurance companies not older than 3 months and local authority tax returns not older than 3 months, or - 1.1.3.2. pursuant to - 152 of the Act on Public Procurement (after the change of the data in the list of economic entities and the addition of this information to the list of economic entities under Section 156 of the Public Procurement Act)

1.1.4 Applicant -of a person in charge of the exercise of proof of personal status pursuant to - 32 par. 1 letter. a) the public procurement law is obliged to submit also a copy of the criminal record for the person who is issued by the General Prosecutor's Office of the Slovak Republic no more than 3 months.

The economic entity entered in the list of economic entities may meet the aforementioned conditions in part to show:

1.1.4.1 . submitting originally- the extract from the criminal record of a legal person not older than 3 months in the public procurement process directly to the public procurer; or 1.1.4.2. by adding this information to the list of economic entities by means of a form of notification of the change of the data, which is published by the Office for Public Procurement (hereinafter referred to as "the office") on its web site with the attached extract from the register-penalties of a legal person not older than 3 months

1.1.5. In the case of a tenderer forming a group of contractors involved in a public procurement, proof of compliance with the conditions of part of the personal status for each member of the group is required. The fulfillment of the condition of part-time according to - 32 par. 1 letter. (e) the act on public procurement proves the member of the group only in relation to that time1.1.6 If the applicant, the place of business or his usual place of residence outside the territory of the Slovak Republic and the status of his place of business, the place of business or usual residence does not issue any of the documents mentioned in - 32 par. 2 of the Public Procurement Act or does not issue or equivalent documents, it may be replaced by a declaration in accordance with the regulations in force in itsplaces of business or usual residence. Conditions of participation of applicants in public procurement related to the financial and economic status and documents for their proofing according to - 33 of the Public Procurement Act

1.1. The bidder shall submit the following original document (s) or his / her certified true copy, by which he / and economic status:

1.1.1.Under - 33 par. 1 letter. (a) the law on public procurement of a bank or branch office of a foreign bank where the beneficiary is open to the account of his ability to execute financial charges for the last 3 calendar years following the proclamation of the public procurement (or for years available from the date of establishment or commencement of activity restraint). The account statement is not considered as confirmationbanks. In the case of an interested person in multiple banks, the contracting authority requests bank information from each of them.

Interested in submitting a statement from a bank, branch office of a foreign bank or a foreign bank in which the holder is guided by an account or an equivalent document include information about: a) the ability of the person to fill financial charges; b) the interest of(c) that the creditor, in case of repayment of the loan, adheres to the due date, d) that his current account is not the subject of execution. I declare honestly that the applicant is only teaching me at the bank (banks) from which he has submitted the bank (bank) statement under this section of the notice.

1.1.3 In accordance with - 33 par. 1 letter. d) the law on public procurement interested in the presentation of the report on the achievement ofin the area covered by the contract, ie in the field of providing services of a similar or similar nature to the subject of the contract for the last 3 marketing years, for the years that are available as of the commencement or commencement of the occupation of the occupant (hereinafter the last 3 business years) by an affidavit of the attorney signed by the statutory body or a member of the statutory body(in this case the interested party shall also submit an original or verifiable photocopy of the power of attorney of that person to represent him / her.)

1.1.4 The assignee must prove that he has achieved in the last 3 financial years the turnover in the area covered by the contract, minimum EUR 14 000 000 excluding VAT. (To clear the turnover achieved, you count other meny The currency of the European Central Bank shall be used for each year.)

1.2. An entrant or an acquirer may use the financial resources of another person, regardless of their legal relationship, to prove their financial and economic status. In this case, an applicant or an engaging person must prove to the contractor that in the implementation of the framework agreement he will actually use the resources of a person whose statusis a means of demonstrating financial and economic status. The fact according to the second sentence is shown by the interested person or the applicant by a written contract concluded with the person whose sources are meant to prove his financial and economic status. From a written contract, the person's handbag must appear to provide performance during the duration of the contractual relationship. The person whose resources are to be used to prove the financesand economic status, must demonstrate compliance with the terms and conditions of the person's personal status in addition to Section 32 1 letter. e) the public procurement law and there shall be no evidence of exclusion pursuant to - 40 par. 6 sheets. (a) to (h) and Article 7 of the Public Procurement Act

1.3.If an applicant can not provide objective evidence to prove the financial and economic status of a documentthe financial and economic standing of the contracting authority may be demonstrated by the submission of another document which the contracting authority considers appropriate. The tender documents for the public tender are published on the day of the announcement of this public procurement in the profile of the contracting authority established in the electronic file on the website www.uvo.gov.sk. The contracting authority will be publicly availableto include in his / her profile any other documents necessary for the preparation of the offer (eg providing explanations pursuant to - 48 of the Public Procurement Act). The contracting authority provides unlimited, complete and direct access to the contract documents and other documents free of charge. I will provide the interested parties also with the basis of a written request sent electronically or by post to:-referred to in section I.1. of this notice. Such a request must clearly indicate the type of public bid to which the application relates (identification of the order by name and of this notice), the identity of the person concerned (business name or name, address, place of business or usual residence of the applicant), contact details of the telephone number and e- mail address. The supporting materials will be sent by e-mail electronicallyto the address specified in the solicitor's request. The contracting authority shall provide an explanation of the information necessary for the preparation of the tender, the application for proof of compliance with the terms of the contract, in accordance with - 48 of the Public Procurement Act and according to point 10 of the tender documents. 4th The public contracting authority requests the commitment to offer security pursuant to Section 46 of the Public Procurement Act. The security of the tender is set at EUR 490 000.00 (forty-eighty euros). - The conditions for the lodging and release of the security are set out in the tender documents. Under Section 66 7 of the Act on Public Procurement evaluation of the fulfillment of the terms and conditions under - 40 of the Act on Public Procurement-the public procurement will take place after the evaluation of the tender pursuant to - 53 of the Public Procurement Act. The public contracting authority shall exclude from the public procurement of the tenderer, in the framework of assessment of the conditions of participation pursuant to - 40 of the Public Procurement Act, if, on the basis of the information provided by me, it is suspected that the tenderer has concluded an agreement in another public entity with another economic entitywith economic outlay.

7. Documents to prove the fulfillment of the conditions of participation of tenderers in public procurement specified in points III.1.1), III.1.2) and III.1.3) of this notice for the award of public service contracts may be replaced in advance by the Contractor pursuant to - 39 of the Act on Public Procurement and the Single European Document (JED), fulfilling the requirements of - 39 par. 2 of the Public Obstaravani, i. including the updated declaration of the economic entity that: a) there is no evidence of its exclusion, b) provides the contracting authority on request with the documents replaced by the JED, the requirements for the execution, the submission of the JED and other details and information related to the JED stated in .............. The contracting authority shall not enter into a framework agreement with an applicant whothe obligation to register in the register of public sector partners under Act no. 315/2016 on the public sector registers and on the amendment of some laws and not entered in the register of public sector partners or subcontractors who are required to register with the public sector partners and are not registered in the public sector register. .
