Subject of the contract: Implementation of an interactive exhibition of the old book and press" Elkamera "as part of the project entitled "Adaptation of the Elbląg Library of a cultural heritage object with tourist potential, stage I", including adaptation of rooms, purchase of equipment and its assembly to display the historical collections of the Libraryeki Elbląska.

The scope of the contract includes the execution of an interactive exhibition of the former "Elkamera" book and press, including the adaptation of the basement rooms of the historic Elbląg Library building and the purchase of equipment and its assembly for the display of collections, including:

a) construction and assembly works;

b) exhibition and exhibition buildings;

c) elements of interior furnishings;

d) preparation of the scenarioopen exhibition;

e) electrical installations;

f) exhibition lighting;

g) teletechnical installations;

h) supply of multimedia technology and devices. Project Sp. z o.o. with its registered office in Krak-w (30-720) at ul. Saska 4/112 contains Annex No.13 to SIWZ.

If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in section VI.3 point 5 item 1 4 submits the documents specified in - 7 cited aboveof the ordinance.

The Employer does not specify the subject condition of participation.

Document submitted by the Contractor at the request of the Employer:

Information of the bank or cooperative savings and credit union, confirming the amount of financial resources or creditworthiness of the Contractor, not earlier than 1 month before the deadline for submission offers.

Note:

a) submitted document jis proof of compliance with the condition of participation specified in section III.1.2 of the notice;

b) in the case of Contractors jointly applying for the award of documents, this or those of the Contractors who on behalf of all will demonstrate compliance with the condition specified in section III.1.2 of the notice;

c) values confirming the fulfillment of the condition given in documents in currencies other than insentenced by the Employer should be converted at the average exchange rate of the National Bank of Poland as at the date of issue of the document, providing the date and exchange rate;

d) if for a justified reason the Contractor cannot submit documents regarding the financial or economic situation required by the Employer, he may submit another document which sufficiently confirms fulfilling the condition of participation in the procedure described by the Employerproceedings.

1. Ref. Section IV.2.6: Minimum period during which the tenderer will be bound by the bid: 60 days.

2. Grounds for exclusion: art. 24 paragraph 1 and 5 points 1 and 8 of the PPL Act

3. In order to initially confirm that they are not subject to exclusion and meet the conditions for participation in the procedure, the Contractors submit together with the offer:

a) the Single European Order Document (EAT), in the case of the Contractor, whichga on the abilities or situation of other entities in order to demonstrate compliance with the conditions for participation in the proceedings, also submits the EAT of this entity;

b) documents constituting evidence, in particular the obligation of entities to provide the Contractor with the necessary resources at the disposal of the contract for the purposes of the contractor, which consists on the abilities or situation of other entities

4. In order to sweatdrilling that Contractors are not subject to exclusion pursuant to art. 24 paragraph 1 point 23 of the Public Procurement Law, all Contractors submit after opening the offers: a declaration of belonging or not belonging to the same capital group.

5. In order to confirm that it is not subject to exclusion, the Contractor whose bid was highest rated submits at the request of the Employer:

1) information from the National RegCriminal ester within the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submission offers, or other document confirming that Wykothe ordinant concluded an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular he obtained the exemption, deferment or installment of overdue payments in installments or withholding in full the execution of the decision of the competent authority;

3) certificate of the appropriate local organizational unit Social Insurance Institutionor Kasa Rolniczego Ubezpieczenia Społecznego (Agricultural Social Insurance Fund) or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has concluded an agreement with the competent authority on repayments these claims together with any interest or interestby fines, in particular, obtained by law exemption, deferment or installment of overdue payments or suspension of the entire implementation of the decision of the competent authority;

4) an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or records to confirm that there are no grounds for exclusion onbased on art. 24 paragraph 5 point 1 of the Public Procurement Law;

5) the Contractor`s statement that he has not issued a final court judgment against him or a final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the case of such a judgment or decision, documents confirming payment of these receivables together with any deductionsdocuments or fines or concluding a binding agreement on the repayment of these debts;

6) a statement by the Contractor about the lack of a decision against him as a preventive measure prohibiting him from applying for a public contract;

7) a statement by the Contractor about the non-payment of local taxes and fees referred to in the Act of 12.1.1991 on local taxes and fees.

In the case ofawcy, which relies on the ability or situation of other entities on the principles set out in art. 22a of the PPL, documents referred to above in item 5. item 1 7 should also be submitted in relation to these entities.
