Main Site: Place of performance of the contract is the territory of the Republic of Poland.

Ordercovers material liability insurance for employees of Miejskie Przedsiębiorstwo Komunikacyjnych Sp. z o.o. in Wroclaw. The coverage will cover approximately 1, 900 people (the number of people may change after the dialogue). Variants of the guarantee sum (guarantee sum per person) will be in the indicative range from PLN 15, 000 to PLN 33, 000 (the sum of the guarantee sumsare subject to change after the dialogue).

I. Contractors who meet the conditions for participation in the procedure regarding the possession of competences or rights to conduct specific professional activities may apply for the award of the contract, insofar as it results from separate provisions, i.e. they carry out insurance activity pursuant to the Act of 11 September 2015 on the activity insuranceand reinsurance (i.e. Journal of Laws of 2019, item 381, as amended), at least to the extent identical to the subject of this contract.

In the case of Contractors jointly applying for the contract in relation to the condition specified above, each of the Contractors acting jointly.

II. Required documents confirming compliance with the conditions for participation in the procedure:

1) permitthe competent authority to conduct insurance activity in the scope identical to the subject of the order or when the permit is not required under separate provisions, a certificate from the competent supervision authority that the Contractor conducts insurance activity in the required scope or when the permit is not required under separate provisions, a statement from the authorized bodyto represent the Contractor that he carries out insurance activity to the required extent and that it is not necessary for him to have a permit together with the legal basis;

2) in the case of the Contractor having its registered office outside the territory of the Republic of Poland, the Contractor additionally confirms the notification received from the supervisory body, and if the supervisory authority in the country of the Contractory does not provide such confirmation, the Contractor submits a statement by the body authorized to represent the Contractor that the notification has been made and accepted by the Polish supervision authority.

III. Information on submitting a declaration along with the offer on the Single European Order Document (EAT) form, which is a preliminary confirmation that the Contractor meets these conditions for participationu in the proceedings, see section VI Supplementary information. Contractors who are not subject to exclusion from the procedure on the basis of the so-called mandatory conditions of art. 24 paragraph 1 of the PPL Act and the so-called optional premise from art. 24 paragraph 5 item 1 of the Public Procurement Law

II. INrequired documents in accordance with the relevant Regulation on the types of documents that the Employer may request from the Contractor, their period of validity and the forms in which these documents may be submitted.

III. The Contractor should attach to the application for admission to the procedure a statement constituting a preliminary confirmation of the date of submission of applications for admission to the procedure.that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure and meets the conditions for selection. The Contractor should make a statement on the form of the Single European Order Document (hereinafter "EAT"). In the case of joint application by the Contractors, the EAT shall submit each of the Contractors applying for the contract.no conditions for participation in the procedure and no grounds for exclusion to the extent that each of the Contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion.

IV. Warning! The application should be submitted under pain of nullity in an electronic form signed with a secure electronic signature verified by means of a valid qualified certificate or equivalentdka that meets the requirements for this type of signature (qualified electronic signature). The application should be made in Polish. The application must be accompanied by EAT in electronic form with a qualified electronic signature.

V. The contractor submits the application using the EIB purchasing platform: https://eib.logintrade.net/u along with EAT, is presented in the "Help" tab, in particular in the FAQ section, available after logging into the EIB purchasing platform. to participate in the proceedings together with

IX. The contractor may rely on the technical capacities of other entities, irrespective of the legal nature of its relations with them. In such a situation, the Contractor is obliged to prove to the Employer that in carrying out the contract he will have the necessary resourcesand to implement the contract, in particular by presenting to this end the undertaking of these entities to make available to him the necessary resources for the purposes of the implementation of the contract.

X. The insurance contracts subject to these proceedings will be concluded and performed with the participation and via the insurance broker EIB S.A.

XI. The awarding entity will choose the best offer based on ito the following assessment criteria (pursuant to Article 60c (1a) of the PPL Act does not provide weight of criteria):

- criterion 1 price,

- criterion 2 optional clauses.
