Task number 1 Insurance:

Compulsory insurance forresponsibility of a civilian entity performing a medical activity.

Voluntary insurance of civil liability for conducting medical activities.

Voluntary insurance of civil liability in respect of the business and property owned. The ordering party must meet the condition of participation in proceedings regarding the competence or authority to conductprofessional activity, in the scope of having a license to perform insurance activity specified in the Terms of Reference, referred to in the Act of 11.9.2015 on insurance and reinsurance activities (unified text Journal of Laws of 2017, item 1170, as amended), and in the event that they started their activity before the entry into force of the Act of 28.7.1990 on insurance activity (Journal of Laws no. No. 59, item 344, as amended), a certificate from the Minister of Finance on having a permit to perform insurance activity.

In order to confirm the fulfillment of the condition regarding competences or entitlements to conduct a specific professional activity, the contractor requests from the contractor: submission of a license to perform insurance activity in Section II, referred to in the Act of 11.9.2015 on insurance and reinsurance activity (uniform text Journal of Laws of 2017, item 1170, as amended), and in the case when they commenced operations before the entry into force of the Act of 28.7.1990 on insurance activity (Dz. U. No. 59, item 344, as amended), a certificate from the Minister of Finance on having consent to perform insurance activities.

The ordering party does not specify the condition forEconomic and financial situation, thus it does not require submitting a document in the above scope.

5.4. Premises for exclusion of the contractor: The Employer will exclude the Contractor from the procedure for awarding the contract on the basis of the reasons specified in art. 24 sec. 1 Pzp.

5.5. According to art. 24 sec. 8 P.z.p. A contractor who is subject to exclusion pursuant to art. 24 sec. 1 point 13 and 14 and 16-20 Pzpand us based on art. 24 sec. 5 points 1-8 PPL, may provide evidence that the measures taken by him are sufficient to demonstrate its reliability, in particular to prove compensation for damage caused by a crime or fiscal offense, financial compensation for damage suffered or remedied, a comprehensive explanation of the facts and cooperation with organsand taking specific technical, organizational and human resources that are appropriate to prevent further offenses or fiscal offenses or improper conduct of the contractor. The first sentence shall not apply if the contractor, being a collective entity, has been the subject of a final court ruling against applying for the order and has not expired.the period of validity of this prohibition specified in this judgment.

5.6. The contracting authority excludes the contractor based on the reasons set out in Article 24 sec. 5 pts 1-8 PzP.

5.7. The Contractor is not subject to exclusion if the awarding entity, considering the weight and special circumstances of the contractor's action, deems sufficient evidence presented on the basis of art. 24 sec. 8 PzP.

5.8. Assessment of compliance with the conditions of participation in postand no grounds for exclusion will be carried out according to the principle meets / does not meet.
