The group of orders of the City and the Limeil-Brevannes CCAS conducts a consultation in order to sign the insurance contracts that make up thewhole market divided into 5 lots.

Insurance.

1) application letter model DC1 (updated version of 26.10.2016) or equivalent;

2) declaration of the candidate model DC2 (updated version of 26.10.2016 ) or equivalent.

1) and 2) DC forms are accessible, not exclusively, on the website http://www.economie.gouv.fr/daj/formulaires;

3) mandate of the insurance company at co(4) mandate of the insurance company to the agent, if any;

5) the power of the person authorized to hire the candidate;

6) for the intermediaries of insurance, the certificate of registration in a register of insurance intermediaries (ORIAS certificate or any other equivalent certificate issued by a body established in a Member State other than France);-insurance and financial guarantee compliant with the Insurance Code (agents and brokers);

8) if the form DC1 is not used, the declaration on the honor that the candidate does not enter in any of the cases of prohibition to tender provided for in Articles 45 and 48 of Order No 2015-899 of 23.7.2015 (model attached to the consultation file)

9) pursuant to Article 49 of Decree No 2016-360 of 25.3.2016, the candidate may submit his application in the form of a Single European Market Document (DUME) instead of the documents referred to in Article 48 of Decree No. 2016-360 of 25.3.2016;

10) When there is grouping, the above documents are to be provided for each member.
