Provision of insurance services for "PKP Intercity" SA in the field of civil liability insurance for the business conducted for 2019 - Reference Number: 08/09 / PRZ / 2018

1 The subject of the order is the provision of insurance services to PKP Intercity SA and in the event of exercise of the right of Option II - for WARS SA with its registered office in Warsaw and Zakład Usług Taborowych "Remtrak" limited liability company with headquarters in Torun in the field of civil liability insurance for the conducted activity.

Civil liability insurance for the activity with a total insurance sum of PLN 10, 000, 000.00 (basic policy). it has been included in the appendices to the ToR.

1contractors who are:

1) are not subject to exclusion based on the grounds indicated in para. 2 below

2) will meet the conditions for participation in the proceedings, referred to in art. 22 ust. 1b of the Pzp Act, ie: -2.1) have competences or entitlements to conduct a specific professional activity, if it results from separate regulations, i.e.: the Employer requires the Contractor to demonstrate the possession of a permit-the competent authority to conduct insurance activities in the scope not narrower than the type and subject of insurance, or if the permit is not required under separate regulations: - a certificate of the competent authority that the Contractor conducts insurance activity in the required scope, or - a statement by the Contractor that it carries insurance activity in the required scope and-it is not necessary to have a permit with reference to the legal basis, or - a declaration of notification of the competent authority of the Contractor's country of residence that the notification has been made and accepted by the competent Polish authority if the Contractor is established outside the territory of the Republic of Poland. for the order of the above-mentioned condition-should demonstrate each of the Contractors jointly applying for the order.

2.2) is in an economic and financial situation enabling the order to be made - (the Ordering Party does not specify requirements in this respect);

2.3) have technical and professional capacity - (The Contracting Authority does not specify requirements in this respect).

2. The Contractor is excluded from the proceedings, which is subject to exclusion onbasis:

1) art. 24 sec. 1 point 12, point 13 lit. a) c) and item 15 of the Public Procurement Law,

2) art. 24 sec. 1 point 14 of the Act, except in the event that the person referred to in this provision has been convicted of an offense listed in art. 24 sec. 1 point 13 lit. d) Public Procurement Law,

3) art. 24 sec. 5 points 1) and 8) of the Public Procurement Law - 3. For more information about the grounds for exclusion, the Purchaser has included in Chapter VIII of the Terms of Referenceon the website from the date of publication of this announcement: www.intercity.pl.

The Ordering Party does not specify in this respect any requirements which the Contractor is obliged to demonstrate in a special way.

1. According to art. 24aa paragraph 1 The contracting authority may first evaluate the offers and then examine whether the contractor whose offer has been evaluated as the most advantageous, not thega exclusion and meets the conditions for participation in the proceedings.

2. The period of binding the Contractor to the submitted offer is 60 days from the deadline for submission of tenders. Information clause from art. The 13th RDO prepared by the Employer for the purpose related to the proceedings for the award of a public order is included in the Specification of Essential Terms of Orders.
