Continuation of works (Stage 3) at the investment - Falticeni Municipal Hospital, Str. Cuza Voda f.n., Falticeni municipality, Suceava County - for authorization of the medical unit according to the IGSU requirements.

The investment follows - the completion of the mining workstricks to investment objective "Construction of Hospital with Polyclinical Dispensary" by executing the works required by IGSU for authorization of the functioning of the medical unit.

Categories of works to be executed within the framework of the contract:

- Temporary work required to carry out permanent works,

- - Resistance, structure and architecture, finishes for:

- Construction of stairs-exterior to Corp A - to the main fa-ade and to the secondary facade - Height P 4, - - Construction of exterior stairs at H - on the main fa-ade - Height P 3, - - Electrical installation of electric current - Lighting and sockets,

- Low voltage electrical installations - detection and signaling, fire alarm, voice-data network, surveillance and alarm systems



- Installation works for limiting and extinguishing fires, outdoor water supply networks,

- Interior finishing works according to IGSU norms,

- Equipment acquisition and installation, commissioning at completion of works

The estimated value according to DG Stage 3 is of RON 3, 771, 354, excluding VAT, set by the designer at Cap. 4.1. DG (HG 28/2008)

1) Failure to comply withincompatibility and conflict of interest as provided by art. 59 and 60 of Law 98 / 2016.

The tenderer (individual / association) must not find himself in the situations provided by art. 59 and 60 of Law 98/2016. Also, according to the provisions of art. 170 and 183 of Law 98/2016, the subcontractors and the third party supporters declared in the tender must not be in the situations provided by art.60. According to the provisions of art. 63 par. (1) of Law 98/2016, the contracting authority mentions at the level of the FDA and within the declaration on its own responsibility the persons with decision-making functions including the person from the auxiliary service provider.

Modality of performance: DUAE presentation by the economic operator in the case of an individual offer, by the economic operators, in the case of joint offer (association), by subcontractors and supporting third parties, if nominated in the tender. With the DUAE, the tenderer (individual / group of operators), subcontractors and / or third-party supporters are presented - Statement on own responsibilityon non-compliance with the situations provided by art. 60 of Law 98/2016; 2) Failure to comply with the exclusion situations provided for in art. 164, 165, 167 of Law no. 98 / 2016.

The bidder (individual / association) must not be in the exclusion situations provided by Law 98/2016 - art. 164, 165 and 167.
