Lease of aircraft for the needs of the Aviation Center of the Higher Vocational School in Chełm

The subject of the order is the lease of the aircraft, which will be used for implementation Practical aviation training for the license of a professional pilot of students in the field of Mechanics and Mechanical Engineering as part of a specializationpilot plane pilotage and helicopter piloting carried out at the ATO Aviation Training Center of the State Higher Vocational School

1. Aircraft type: Tecnam P2006T, be equivalent. Multi-engine airplane, piston (land) MEP (L). Equipped with retractable undercarriage,

With adjustable propeller, minimum four-person. The minimum power of the engineand 100 hp. Produced min. 2014, must have a functional ELT rescue transmitter permanently mounted to the airframe construction. In the absence of such a transmitter, the owner of the aircraft must provide access to the ELT mobile rescue transmitter during the time the airplane is operated by PWSZ

2. The base airport: Lublin (EPLB); Depułtycze Krolewskie (EPCD)

3. Technical supervision: The aircraft must be managed in the organization of Continuing Airworthiness Management. The CAMO organization in which the owner's aircraft is managed must provide training in the operation of the On Board Technicalnel chosen by the PWSZ in Chełm. The training will be carried out on the day of transfer of the aircraft. A protocol will be drawn up on the transfer of the aircraft. The owner of the aircraft is obliged to carry out technical services at his own expense, in the area of the Airport in Krolewskie Depułtycze in the event of technical malfunction or maintenance work, lasting longer than 7 days, the SP owner must provide a substitute vessel with the same parameters. In the event of prolonged maintenance work on the aircraft (technical failure of the SP, waiting for parts), the owner may request in writing for an extension of the service deadline.

Insurance: the owner of the aircraft is required to present the current insurance policy (Aerocasco, OC, NNW) and include in the OC policy of the aircraft operator PWSZ in Chełm; The scope of insurance must include:

4. Supervising instructor: OKL PWSZ wChelm;

5. Responsibilities of the owner: the owner of the aircraft is required to enter in the registration certificate, the PWSZ in Chełm as the aircraft user. The aircraft will be delivered to the place of training based on the training plan transmitted to the landlord for a given month by the Center's HT Training Manager.

6. Aircraft Audit: The Ordering Party reserves the right to a reviewan aircraft audit and inspection of the aircraft documentation by authorized employees of OKL PWSZ in Chełm, before and during the contract signing. 7. The number of hours of bombing: up to 600 h

8. The period of order fulfillment: until December 31, 2020, with the possibility of extending the deadline for order fulfillment in case of adverse weather conditions or due to circumstances affectingh for the implementation of the student education process (eg failure to meet the statutory requirements of the training participant to proceed to the next stage of aviation training),

9. The orderer provides fuel and oil for 600 hours of flight, the orderer does not provide fuel and oil for service.

A) Comments: the aircraft will be handed over and picked up with a full fuel tank

The ordering party does not putdetailed requirements for the description of compliance with this condition of participation in the proceedings - In order to confirm the lack of grounds for exclusion from the proceedings referred to in point 5.1. SIWZ and to confirm compliance with the conditions for participation in the proceedings referred to in point 5.2. The Contractor will be required to provide the Employer with the following statements and documents (on dates indicated in SIWZ):

a) Contractor's statement constituting preliminary confirmation of fulfillment of the conditions for participation in the procedure set out in the ToR and no grounds for exclusion, submitted on a single European order document (hereinafter referred to as "EAT"), modeled by the Commission Implementing Regulation (EU) 2016 / 7 from 5.1.2016 establishing the standard Single European Order Document (Dz.Urz. UE L series 2016, No. 3, page 16). The content of EAT has been specified in Appendix No. 3 to the ToR, however, due to the need to provide a significant amount of information in the content of EAT, the Employer recommends using an editable version of this document at https://www.uzp.gov.pl/baza-wiedzy/ single-European-document-order

EAT should be submitted with the offer;

b) documents confirmclaiming that the Contractor is insured against civil liability in respect of the business related to the subject of the order for a guarantee sum in the amount of at least PLN 200, 000.00 (say: two hundred thousand zlotys) indicated in the EED to confirm the condition of participation in the economic situation or c) a copy from the relevant register or from the Central Register-Records and Information on Business, if separate regulations require an entry in the register or records to confirm the absence of grounds for exclusion under Article. 24 sec. 5 point 1 Pzp;

d) certificate of the competent head of the tax office confirming that the Contractor is not in arrears with paying taxes, issued no earlier than 3 months before the offer submission deadline expirest or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment in installments of overdue payments or suspension in full of the decision of the competent authority; e) a certificate of the relevant local unit orgathe Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these amounts(f) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act and, regarding the conviction for a prison sentence, in the scope ofeated by the Ordering Party on the basis of art. 24 sec. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting offers.
