    Implementation of comprehensive cardiac rehabilitation service under the project: "Cardiological rehabilitation as an opportunity to extend the professional activity of the inhabitants of the Pomeranian Voivodeship"

    The object of the contract is the implementation of compex cardiac rehabilitation service under the project: "Cardiac rehabilitation as an opportunity to extend the professional activity of the inhabitants of the Pomeranian Voivodship", co-financed by the European Union under the Regional Operational Program of the Pomeranian Voivodship for 2014-2020 (procedure: open tender). Detailed description of the order , rangeand the manner of its implementation is specified in the following annexes to the ToR: Annex 1A - Price form, Annex 1B - Description of the Object of the Order (OPZ) together with the Regional Health Program - Cardiological Rehabilitation Module (Annex 1 to OPZ), constituting OPZ, Annex No. 1C - Draft contract.
    Main Site: Area of the Lębork poviat
    Provision of servicescomplete cardiac rehabilitation (as described in Annex No. 1B to the ToR - OPZ) in the Lębork poviat for:
    1) at least 500 people of working age living in the Pomeranian Voivodeship with a confirmed cardiovascular disease who did not have a cardiological incident , according to the following schedule:
    a) min 130 people in 2019
    b) min 160 people in 2020
    c) min. 160 people in 2021
    d) min. 50 people in 2022
    2) a minimum of 170 people in the age of professional activity residing in the Pomeranian Voivodship, with a confirmed cardiovascular disease, after a cardiac incident, according to the following schedule :
    a) min 40 people in 2019
    b) min 60 people in 2020
    c) min 55 people in 2021
    d) min 15 people in 2022
    Comprehensive cardiac rehabilitation in the field ofmedical services. Cardiac rehabilitation is a comprehensive program covering physical training, modification of risk factors, education and psychological support. In the scope of services rendered, the Contractor (Implementer) will be required to perform the following tasks: recruitment of Program participants, inclusion of a patient in the Program, appointment of a Patient Caretaker, implementation of a comprehensivecardiac rehabilitation (diagnostic tests, therapeutic sessions), termination of participation in the Program, monitoring, testing the quality of services in the Program, operation of the internet platform, sharing and distributing information and educational materials, participation in training sessions, events, "white days", keeping and storing documentation associated with the implementation of the Program on each languageego stage. A detailed description of the subject of the contract is specified in Annex 1B to the ToR. Description of the Subject of the Order (OPZ).     c) The Contractor`s statement that no decision has been issued against him as a preventive measureapplying for public procurement - annex 5 to the ToR; Certificate of entry of a medical entity into the register of entities performing medical activity, list of persons - Annex No. 6 to the ToR, list of tools, plant equipment or technical devices - Annex No. 7 to the ToR.
    Documents submitted by foreign Contractors: the Contractor having its registered office or place of residence outside the territory of the Propertyof the Republic of Poland instead of the documents referred to in - IX para. 3 point a SIWZ (Information from the National Criminal Register), submits information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence. that they concerny information or document, to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act. The Awarding Entity informs that it may first evaluate the offers and then examine whether the Contractor whose offer has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure (Article 24aa (1) of the PPL). A condition of participation in the contract award procedure is a bid security of PLN 1,000.00. The bid bond must remain nsuspended before the deadline for submission of tenders and must be granted by the end of the period of validity of the offer. 93 item 1a of the Public Procurement Law
