Provision of basic care services in the place of residence in the city of Nowy Sącz for the years 2018-2021

Comprehensive provision of basic care services in the place of residence in the New Town Sacza for 2018 2021, hereinafter referred to as "services" or "care services" in the place of residence on terena of the city of Nowy Sącz from November 1, 2018 to December 31, 2021. The Contractor will perform services in the city of Nowy Sącz, in the place of residence of the person using the services or other place indicated by the Ordering Party. The Contractor will perform the range of care services that results from the needs of the persons using the assistance and is determined individually depending on the needs, evenif this range will differ from the specification or contract. The Employer guarantees the number of hours (1 hour is 60 minutes) of basic care services in 2018 2021:

Comprehensive provision of basic care services in the place of residence in the city of Nowy Sącz for 2018 2021, hereinafter referred to as "services" or 'care services"in the place of residence in the city of Nowy Sącz from November 1, 2012 to December 31, 2021. The contractor will perform services in the city of Nowy Sącz, in the place of residence of the person using the services or other place indicated by the ordering party. caring which results from the needs of the persons using the assistance and is determined individuallye depending on the needs, even if the scope will differ from the specification or contract. The Employer guarantees the number of hours (1 hour is 60 minutes) of basic care services in 2018 2021:

a) The minimum number of service hours is 160, 000 (in words: one hundred sixty thousand hours)

b) The maximum number of service hours is 180, 000 (in words: one hundred and eighty thousand godzin). Detailed obligations of the Contractor or a person authorized by him are specified in the model contract constituting Annex 8 to the relevant conditions of the order. The contract constituting Annex No. 8 to relevant conditions of orders specifies further general and specific requirements, in particular regarding the rules and deadlines, the manner of performing activities and obligations of the Contractor and persons performing in his nametion of activities resulting from the contract for the provision of services
