The subject of the order is mowing grass surfaces in road lanes of the city of Radom in 2020, area V.
Main Site: Radom, POLAND
The object of the contract is mowing grass surfaces in road lanes of the streets of Radom in 2020 years of the V region.
The contract includes:
- mowing of grass surfaces 750, 000 m,
- removal and grubbing up of self-seeding plants 1, 000 m. b) offer cost estimate constituting Annex 2 to the ToR;
c) draft contract Annex 3 to the ToR.
In order to initially confirm complianceand the conditions for participation in the procedure and the lack of grounds for exclusion, the Contractor submits with the offer a statement on the form of the Single European Order Document ("EAT").
To confirm the absence of grounds for exclusion from the procurement procedure in the circumstances referred to in art. 24 paragraph 1 point 23 of the Pzp, a declaration of belonging or not belonging to te the capital group itself within 3 days from the day of posting on the website. Employer`s information specify. in art. 86 section 5 Public Procurement Law
At the request of the Employer, the Contractor shall submit:
1. an excerpt from the relevant register or from CEIDG, if separate provisions require an entry in the register or a record for purposes of confirmation. no basic Unmark. based on Art. 24 paragraph 5 point 1 Pzp;
2. certificate of the competent head of administrationS confirmation that he is not in arrears with taxes, issued no earlier than 3 months before the deadline for submitting bids or another confirmation document. that the Contractor has entered into an agreement with the relevant tax authority regarding the repayment of these receivables together with interest or fines, in particular he obtained the exemption provided for by law, deferment or distribution into installments of overdue payments orfull compliance with the decision of the competent authority;
3. certificate of the appropriate local organizational unit of ZUS or KRUS or other document confirming that it is not in arrears with the payment of insurance premiums. social or health, issued not earlier than 3 months before the deadline for submitting bids or other document confirming that he has entered into an agreement with the competent authority on repaymentt of these receivables, together with any interest or fines, in particular, obtained legal exemption, deferment or distribution in installments of overdue payments or withholding in full of the decision of the competent authority;
4. information from the KRK regarding Art. 24 paragraph 1 point 13, 14 and 21 of the PPL and regarding conviction for an offense against imprisonment, in the scope of defining based on Art. 24 paragraph 5 points 5 and 6 Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids;
5. statement that he has not issued a final court judgment or final admission decision on late payment of taxes, fees or insurance premiums social or health or - in the case of such a judgment or decision - documents confirming payment of these claimstogether with any interest or fines or the conclusion of a binding agreement on the repayment of these debts (in relation to Article 24 paragraph 1 item 15 of the PPL);
6. a statement that he has not been issued a decision as a precautionary measure forbidding public procurement (in relation to Article 24 paragraph 1 item 22 of the PPL);
7. statement that no final conviction has been issued forviolation of the penalty of restriction of liberty or a fine to the extent specified pursuant to art. 24 paragraph 5 points 5 and 6 of the Pzp;
8. statement that no final admission decision has been issued on breach of obligations arising from provisions of labor law, environmental protection or social security provisions. to the extent specified pursuant to art. 24 paragraph 5 point 7 of the Pzp;
9. declaration of non-payment afterlocal donations and fees, referred to in the Act of 12.1.1991 on local taxes and fees.
The offer must include:
1. offer form with the cost estimate;
2. power of attorney, if required;
3. third party obligation or other document serving to demonstrate access to the Contractor`s potential;
4. EAT as the Contractor`s own statement;
5. EAT for each of the entities on zasobach which the Contractor relies on;
6. EAT for each of the Contractors jointly applying for the contract;
7. bid bond in non-monetary form in the form of an electronic original of the bid bond document with a qualified electronic signature.
