PThe object of the orders is the construction and implementation of a computer system of supervision over the water supply network and sanitary sewer network of the city of Rzesz-w.

Main Site: 35-055 Rzesz-w

The aim of the orders is to implement an integrated IT tool in the Employer's enterprise serving, among others, to: - optimize the operation of the water supply and sewage system with the support of toolsfor its modeling and monitoring (SCADA), based on a dedicated application made on the basis of a professional SCADA system, - management of water and sewage assets, - support of the customer service process, - support of planning and strategic decision making.

The Ordering Party does not specify in this respect any requirements that the Contractor is required to demonstrate in order tospecial item.

1. The Contractor together with the offer shall submit the current: - 1) Declaration, in the form of a Uniform European Order Document (EIN), constituting preliminary confirmation that the contractor is not subject to exclusion and fulfills the conditions for participation in the proceedings - the Employer requires that the Contractors fill in Part II, III, IV a, and VI. GOODS

The Ordering Party informs that the details of infRegulations regarding the Single European Order Document (EAT) can be obtained on the website https://www.uzp.gov.pl/baza-wiedzy/prawo-zamowien-publicznych-regulacje/prawo-krajowe/jednolity-europejski-dokument-zamowienia.

2) Obligation of the entities to put at the disposal of the necessary resources for the needs of order fulfillment, if the Contractor relies on the abilities or situation andn the entities to prove to the orderer that while executing the order, the Contractor will have the necessary resources of these entities

3) The Bid Form prepared on the basis of Annex 1 to the Terms of Reference and the Annex to the List of Key Equipment (WKU) prepared on the basis of Attachment No. 1A to SIWZ,

4) The power of attorney must be attached to the offer in case the offer is made on behalf of the contractorputs a proxy or when the offer is submitted by contractors jointly applying for the order,

2. In the case of joint application for contractors, JINS shall submit each of the contractors jointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion in the scope of, which each of the contractors demonstrates fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion. EAT each contractor jointly applying for the order must have an electronic form with a qualified electronic signature by persons authorized to represent each of these contractors.

3. In the case of contractors who to confirm the fulfillment of thigh conditionsin the proceedings rely on the resources of other entities, on the principles set out in art. 22a Pzp, in order to demonstrate the lack of grounds for exclusion and fulfillment of conditions for participation in the proceedings - in so far as they refer to their resources - the contractor also submits to the ordering authority the EAT of these entities; EAT OTHER entities must have an electronic form with a qualified afterelectronically by persons authorized to represent each of these entities, - the Contractor must demonstrate that

a) has financial resources or creditworthiness, in an amount not less than PLN 1, 500, 000.00 (in words: one million five hundred thousand zlotys); b) he is insured against civil liability in respect of his business related to the subject of the order for an amount of noless than PLN 1 500 000.00 (in words: one million, five hundred thousand zlotys),

1. Contractors who are not subject to exclusion and meet the conditions for participation in the procedure, specified by the ordering party in the notice of procurement and the ToR, may apply for the award of the contract.

2. The Awarding Entity will exclude a contractor from the proceedings in the circumstances indicated in art. 24 sec. 1 pts 13-23 Pzp., Will take into accountthe provisions of Article 133 ust. 4 PzP.

3. The contracting authority does not provide for the exclusion of the contractor from the procedure based on art. 24 sec. 5 Pzp.

4. The ordering party prior to the order being awarded (ie before selecting the best offer) will call the contractor whose offer has been rated the most, to submit within the prescribed period, not shorter than 10 days, valid as of the day of submitting statements or dokumentow confirming:

1) no grounds for exclusion:

a) Information from the National Criminal Register in the scope specified - in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law, with the exception of the case referred to in art. 24 sec. 1 point 13 lit. d of the Public Procurement Law, and in the case referred to in art. 24 sec. 1 point 14 of the Public Procurement Law, if the person referred to in this provision has been convicted of an offense listedArt. 24 sec. 1 point 13 lit. d., issued not earlier than 6 months before the deadline for submitting offers;

b) Declaration of the contractor on the lack of issuing a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance contributions, or - in the case of issuing such a verdict or decision of documents confirmingmaking the payment of these claims together with any interest or fines or entering into a binding agreement regarding the repayment of such claims; c) Declaration of the contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders;

2) fulfillment of the conditions in the proceedings: a) Information of the bank or Cooperative Savings and Credit Unionconfirming the amount of funds owned or the creditworthiness of the contractor, in a period not earlier than 1 month before the deadline for submitting bids,

b) Document confirming that the contractor is insured against civil liability in respect of the business related to the subject of the order, c) List of deliveries completed in the last 5 years before the end ofm the deadline for submission of bids, and if the period of business is shorter - during this period, with their value, subject, dates of completion and entities for which deliveries were made, and attaching evidence determining whether these deliveries were made, the evidence referred to are references or other documents issued by the entity for whose suppliesy were performed, and if for a justified reason of objective nature the contractor is not able to obtain these documents - a statement by the contractor;

d) List of persons referred by the contractor to perform the public order, in particular responsible for providing services, quality control or management of robots construction, along with information on their qualifications for the professionrights, experience and education necessary to perform public order, as well as the scope of activities performed and information about the basis to dispose of these persons.

3) deliveries meet the requirements specified by the contracting authority: a) catalog cards or other documents confirming the fulfillment of required by the Ordering Party, the technical parameters of devices ongiven by the Contractor in the List of Key Equipment (WKU),

5. In the case of Contractors jointly applying for the award of the above-mentioned tolerated aspect. and doc. from point 4. 1) each of the Contractors jointly appears.

6. In the case of another entity on whose capacity the contractor relies, the Contractor shall submit the above mentioned statements and documents from point 4. 1) in respect of each of these entities.
