Reconstruction of heating networks in the area of Legionowa, Poleska, Palowa, Chrobrego and Bohaterow Monte Cassino

Reconstruction of heating networks divided into tasks:

Reconstruction of the heating network between ul. legionary, Suraska, Rynek Kościuszki from the AK-310 chamber to the AK-35 chamber, from the AK-34 chamber to the AK-33 chamber with connections to the buildings at ul. Suraska, Rynek Kościuszki.

In order to confirm compliance with this condition, the Contractor should show civil liability insurance in the scope of activities related to the subject of the order.

At the request of the Employer to confirmcompliance of the Contractor with the conditions of participation in the proceedings, the Contractor will be required to submit the following declarations or documents: - a document confirming that the contractor is insured against civil liability in respect of the business related to the subject of the order for a guarantee sum determined by the ordering party. The proceedings are conducted in Polishon the electronic Logintrade Purchase Platform.

2. In the present proceedings, all documents, statements, applications, notices and information of the Ordering Party and Contractors are only provided in electronic form via the Logintrade Purchase Platform.

3. The Contractor is obliged to submit a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractorit is not subject to exclusion and meets the conditions for participation in the proceedings.

4. In the scope of Part IV of the Single Document, the Contractor fills only the section "α".

5 The Contractor who refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that they refer to their resources, conditions of participation in the proceedings, submit a single documenty also regarding these entities. In the scope of Part IV of the single document, only section "α" should be completed.

6 In the case of joint application by Contractors, a single document is submitted by each of these Contractors.

7 If the Contractor is established or resides outside the territory of the Republic of Poland, and also when the person to whom the document relateshas a place of residence outside. The territory of the Republic of Poland, the principles set out in point 7.8-7.10 of the Terms of Reference - 8 apply. Contractor, within 3 days from the date of posting on the website the information referred to in Article 86 para. 5 of the PPL, shall provide the Ordering Party with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23Public Procurement Laws

9. The contractor is excluded from the contract award procedure in respect of which any of the circumstances referred to in Article 24 sec. 1 point 12-23 of the PPL Act and 24 sec. 5 points 1, 2 and 8.

10. The ordering party, in accordance with art. 24 aa of the Public Procurement Law, will first evaluate the offers, and then examine whether the contractor whose offer was evaluated as the most advantageousand is not subject to exclusion and meets the conditions for participation in the proceedings.

11. At the request of the Ordering Party, in order to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings, the Contractor is obliged to submit information, certificates, excerpt and statements referred to in - 5 points from 1 to 6 and 9 of the Regulation of the Minister of Development dated 26.7.2016 on the types of documents, jakich may be requested by the contracting authority from the contractor in the procedure for awarding the contract (Journal of Laws of 2016, item 1126 as amended).
