The subject of the contract is "Implementation of forest management works in the Gorczański National Park in 2020-2021" The subject of the contract includes the performance of works in the field of forestry services in the Gorczański ParkNational in 2020 2022 and was divided into 6 partial tasks. The material scope of the subject of the contract includes the total amount of works in the field of forest cultivation and protection, non-forest ecosystems, the protection of rare animals, protection of cultural heritage, tourism, logging and logging.

List and brief description of conditions:
1. Contractors who can apply for the contract who:
1) are not subject to exclusion pursuant to art. 24 section 1, however, the contracting authority also provides for the exclusion of the contractor:
a) in relation towhose liquidation has been opened, the arrangement approved by the court in the restructuring proceedings provides for satisfaction of creditors by the liquidation of its assets or the court ordered the liquidation of its assets pursuant to art. 332 paragraph 1 of the Act of 15 May 2015 Restructuring Law (Journal of Laws of 2019, item 243) or whose bankruptcy was announced, except for the contractor who after the announcemententered into an arrangement approved by a final court decision, if the arrangement does not provide for satisfaction of creditors by the liquidation of the assets of the bankrupt, unless the court ordered the liquidation of his assets under art. 366 section 1 of the Act of February 28, 2003 Bankruptcy Law (Journal of Laws of 2019, item 498);
b) who grossly violated his professional obligations, which undermines his integrity, inof the contractor if the contractor, as a result of the intended action or gross negligence, failed to perform or improperly performed the contract, which the contracting authority is able to demonstrate by appropriate means of evidence;
c) which, for reasons attributable to it, failed to perform or improperly performed significantly earlier public procurement contract or concession contract concluded withaviating persons referred to in art. 3 clause 1 item 1-4 of the Public Procurement Law, which led to the termination of the contract or the award of damages.
The Employer does not set a detailed condition in this respect
The Employer will call in accordance with art. 26 section 1 of the Public Procurement Law, the Contractor whose tender was highest rated to be submitted within the time limit set for not shorter than 10 days as at the date of submitting the declarations ordocuments confirming the circumstances, i.e. no grounds for exclusion from the procedure and confirmation of compliance with the conditions for participation in the procedure. Editable form templates (Annex 5, 6, 7 and 8 to the Terms of Reference) will be attached to the request. The documents / statements are:
1) list of persons referred by the contractor to perform the public contract, together with information on their qualificationsprofessional information, authorizations, experience and education necessary to perform the public contract, as well as the scope of activities performed by them together with a declaration of possession. A template of the list is Annex 7 to the ToR;
2) list of tools, plant equipment or technical devices available to the contractor for the performance of the public contract with information on the groundsie to dispose of these resources. A template of the list is attached as Annex 8 to the ToR;
3) an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;
4) a statement that there are no prerequisites for the Contractorand referred to in art. 24 paragraph 5 point 4 of the Public Procurement Law, i.e. (...) which, for reasons attributable to it, did not perform or improperly significantly performed the previous public procurement contract or concession contract concluded with the awarding entity referred to in art. 3 clause 1 item 1-4 of the PPL, which led to the termination of the contract or the award of damages (the model statement providesAnnex 5 to the ToR);
5) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submission of bids;
6) certificate from the competent head of the tax office confirming that the contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submissionand the offers or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full the execution of the decision of the competent authority;
7) certificate of the competent field unitorganizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund (Kasa Rolniczego Ubezpieczenia Społecznego) or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids or other document confirming that the contractor has concluded an agreement with the relevant repayment authoritythose receivables, together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or distribution in installments of overdue payments or withholding in full the execution of the decision of the competent authority;
8) the contractor`s declaration that he had not issued a final court judgment or final administrative decision regarding him arrears with taxes, fees or pcontributions to social or health insurance or in the event of such a judgment or decision, documents confirming payment of these claims together with any interest or fines or conclusion of a binding agreement on the repayment of these claims (in relation to the condition of exclusion described in Article 24 paragraph 1 point 15 PZP) (according to the model constituting Annex 6 to the ToR);
9) the contractor`s statement that he has not been subjected to a preventive measure against the prohibition of competing for public contracts (regarding the exclusion condition
