1. The subject of the order is operational leasing of 10 passenger cars for needsSpołki AMW Sinevia sp. Z oo

The Contractor will be obliged to deliver 10 passenger cars under operating lease for a period of 36 months with the right to buy at 1% at the end of the contract.

Detailed description of the subject of the order is included in the Description of the subject - Annex No. 1 to SIWZ.

2. The place of delivery of the order is: AMW Sinevia Sp. z o.o., ul. Paderewskiego 6, 05-100 Nowy Dwor Mazowiecki

3. The terms of order execution are contained in the Description of the subject of the order, Annex 1 to the Terms of Reference and the draft Contract constituting Annex No. 5 to the ToR.

4. The ordering party requires a guarantee: for mechanical parts / varnish min. 24 months or 100, 000 km (whichever comes first)

5. Szczegiel includes Annex 5 to the draft contract.

1. Contractors, who are not subject to exclusion pursuant to Article 24 para. 1 point 12-23 of the Public Procurement Law and art. 24 sec. 5 point 1 of the Public Procurement Law

2. For the purpose of preliminary confirmation that the Contractor is not subject to exclusion, he must submit a statement, valid as of the day of submitting bids, submitted in the form of a Single European Order Document (EIN) Annex No. 4 to the SIWZ, to the extent indicated by the Awarding Entity in the contract notice and SIWZ.Informacje contained in the EO will constitute a preliminary confirmation that the Contractor is not subject to exclusion - prepared in accordance with the Form of the Single Order Document constituting Annex No. 2 to Commission Implementing Regulations (EU) 2016 / 7 of 5.1.2016 establishing a standard form for a European document-orders (Official EU L 3/16) to the extent indicated in Annex 4 to the ToR.

3. In case of joint application by the Contractors of the ERO, referred to in point 2, each of the Contractors jointly applying for the order. This document is to confirm the lack of grounds for exclusion to the extent that each of the Contractors shows no grounds for exclusion.

4. The contractor that he intends to entrustexecution of a part of the orders by subcontractors, in order to demonstrate the lack of grounds for exclusion of participation in the procedure, submit the EAG referred to in point 2 to these subcontractors.

5. The ordering party, prior to the order being awarded, confirming the absence of grounds for exclusion, will call the Contractor whose offer has been rated the most, to submit in the shortened period, not shorter than 10 days, withinas of the day of submitting the documents listed in section III.1.2), ie:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids;

2) excerpt from the relevant register or from central records and information on economic activity, if separate regulations require into the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law Act or will not call if it is obtained by means of free and publicly accessible databases of public registers;

3) the Contractor's statement that he has not issued a final court decision or final administrative decision about his tax arrears, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming payment of these receivables together with any interest or fines or conclusion of a binding agreement on repayment of these claims, is attached as Appendix 6 to the ToR;

4) statements by the Contractor about the lack of a ruling against him in the middle ofa preventive prohibition of applying for public procurement is provided in Appendix 6 to the ToR,

5) if the Contractor intends to entrust the execution of a subcontractor's part (which is not an entity where the Contractor relies on the principles specified in Article 22a of the Act Pzp) documents listed in point 1) 4).

6. If the Contractor has his registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to above - in Section III.1.2):

1) subpoint 1) - he shall submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the contractor has its registered office or place inthe person or place of residence has a person or place of residence, to the extent specified in art. 24 sec. 1 item 13, 14 and 21 of the Public Procurement Law,

2) sub-point 2) - submits a document or documents issued in the country in which the contractor has its registered office or place of residence, confirming that: a) no liquidation has been opened or bankruptcy has been announced .

7. Documents referred to in point-6 point 1) and item 2) lit. a), should be issued not earlier than 6 months before the deadline for submission of tenders. If in the country in which the Contractor has his registered office or place of residence, the documents referred to in item 6 are not issued, they are replaced by a document containing a statement from the Contractor, indicating the personsy or persons authorized to represent it, or a statement of the person to whom the document was to be applied, submitted to a notary or a judicial, administrative or professional self-regulatory or economic body competent for the Contractor's seat or place of residence or place of residence. The provision of para. 7 shall apply accordingly - 9. Contractor based in territoryorium of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to which the document referred to in point 5 sub-item 1 applies) - in Section III.1.2), submits the document referred to in point 6 (1), in the scope specified in art 24 sec. 1 point 14 and 21 of the Public Procurement Law. If in the country where the person whose document was to reside is in the place where he / she resides, nosuch documents are issued, it is replaced by a document containing a statement of that person made before a notary or before a judicial, administrative or professional self-government or economic authority competent for the place of residence of that person. The provisions of point 7 shall apply accordingly.

1. The contractor will be bound by the offer for a period of 60 days. The period of binding ofemercury begins at the end of the deadline for submitting bids.

2. The Contracting Authority informs that in the present proceedings Article 24aa paragraph 1 of the Public Procurement Law, ie the Contracting Authority will first evaluate the offers and then examine whether the Contractor whose tender has been rated as the most advantageous is not subject to exclusion. The Contractor joining the proceedings is obliged to pay a bid bond in the amount of:-PLN 8, 000.00, in the form and according to the rules specified in accordance with art. 45 PPL Acts
