Region of North Jutland offers a framework agreement-on financial leasing. The offered lease frame is estimated at DKK 400, 000, 000 and is intended to finance k0b, including special medical equipment. The offer is a public tender. Tenderers who are covered by one of the exclusions used in the offer or who do not meet the Northern Jutland Minimum Requirements for Fitness (Technical Form) can not be considered.the Common European Procurement Document (ESPD) as proof that the tenderer is not subject to exclusion and meets the minimum eligibility requirements. The ESPD is attached to the tender document as an XML document (Appendix E).

A bargaining bidder constitutes an association of companies (ie, where several companies merge as consortium), a separate ESPD must be completed and sent to each company in the association. There are no specific requirements for the legal form of the association, but a joint person or company must be designated among the association's companies in alles is authorized to subscribe to all companies in the association. In addition, companies in the association must be jointly and severally liable, directly and unconditionally for the performance and performance of the contract. As part of its documentation for the information contained in the ESPD, the Association shall submit a completed Annex F (Declaration of Joint Authorizing Officer in Consortia), cf. Section 11 of the Terms of Service.

The tenderer may base himself on the technical and professional form of other entities, irrespective of the legal nature of the relations between the tenderer and the entity (s) on which the tenderer is based. If the tenderer is based on the technical and professional form of other entities, the tenderer must submit supporting statements or other evidence confirming that, as part of his documentation for the information contained in the ESPD, This actually lines the necessary technical and professional form.

In addition, -The offer includes a completed ESPD for the bidders themselves and a separately filled ESPD for each of the units on which it is based.

The tenderer must provide the ESPD with the information requested in Parts II, A, B and C.

Tenderers must provide in ESPD Part III A, B and C information regarding the mandatory grounds for exclusion set out in section 135 and 136 of the Public Procurement Act.ESPD Part III C provided information regarding the voluntary grounds for exclusion set out in section 137 (1) of the Public Procurement Act. 1, Nos. 2 and 3.

Unless, at the request of the contractor, sufficient documentation of legality is provided, cf. Section 138 of the Public Procurement Act, tenderers excluded from the above grounds for exclusion are excluded.

ESPD Part IV, C shall be filled in with the bekendtg0relseSection III.1.3 Requested Information.

In ESPD, Part II, D is required to indicate whether the bidder intends to use subcontractors for the execution.

If this is the case, the tenderer must indicate which parts of the contract the tenderer intends to provide in subcontracting to third parties, as well as which subcontractor the tenderer proposes if he or she is known at the timeone for the delivery of the ESPD document.

ESPD Part V and Part VI shall not be completed or signed unless the contracting authority so requests.

As evidence of grounds for exclusion: - An excerpt of relevant register or equivalent document issued by a competent authority showing that the tenderer is not covered by the grounds for exclusion in Section 135 (3) of the Public Procurement Act. 1, and

- A certificate issued by a competent mineevidence that the tenderer is not covered by the exclusion grounds in section 135 (1) of the Public Procurement Act. 3 and 137 (1). 1, No. 2 and No. 3.

For Danish tenderers, the documentation requirement can be fulfilled upon presentation of a service certificate that must be issued no more than 6 months before the date of submission. Be sure to ensure that the Service Certificate contains information about the above mentioned exclusivelyseasons.

As documentation for the technical and professional form:

There is no requirement for further information, including in the form of a real reference list, in addition to the information provided in the ESPD.

The contracting entity is, however, entitled to request a signed reference list, containing the most significant contracts of equivalent nature performed by the tendererFor the last 3 years, see the descriptions in the Exclusion, Appropriateness and Guidance for ESPD (Annex D), Section 1.4.3. In that case, a reference list will be submitted as requested.

The contracting entity reserves the right to contact the references provided for confirmation of the accuracy of the information.
