1) The subject of the order are works consisting in modernization of road lightingin the City and Commune of Chmielnik (voivodship Świętokrzyskie) with the use of new LED luminaires integrated with an IT system allowing control and monitoring of road lighting equipment. The subject of the orders also includes design works that will be implemented before the robot is taken. Modernized fixtures will be mounted on existing polesh low voltage power lines and dedicated poles. Modernization works include dismantling of functioning luminaires with booms and uninsulated power cables, which will be replaced with new LED luminaires with booms. In the scope of works there is also a suspension of insulated conductors (AsXSn 2x25mm2) and assembly of new independent power supply and control boxesch together with control and protection devices for road lighting circuits. The implementation of the task includes designing and then performing the modernization of road lighting.

2) The proceedings are conducted in one part but divided into two stages:

a) Stage I Modernization of lighting in the Chmielnik commune

The contractor is obliged to carry out the modernist projectlighting arrangement covering all elements described in the attached functional and operational program for the Chmielnik Communal Area I based on the agreed technical design will carry out the planned delivery with assembly.

It is planned:

- disassembly of 1673 existing fittings equipped with gas discharge light sources various degree of wear along with disassembly of the accompanying osprof the tooth, booms, uninsulated conductors;

- assembly of 1384 pcs of new LED luminaires, including assembly of 114 pieces of luminaires in a new location on existing poles of low voltage power network, as a complement to road lighting and assembly of new equipment, among others control and measuring boxes, and in the case of overhead lines, insulated conductors and booms* After disassembling the existing fittings, luminaires with LED sources will be installed in their place. In the case of overhead networks, a boom should be installed in accordance with PFU. Optimization of the luminaire suspension geometry is achieved through the use of luminaires with adjustable tilt angle for obtaining the best lighting effects, including the compliance of the parameters with the PN-EN 13201 standard.dishes are provided for the elevation of control points and electricity settlement from the closed facilities of the Power Distribution System Operator, ie PGE Dystrybucja S.A. for positions that do not limit accessibility. The elevation of control points must take into account the Employer's guidelines regarding the reduction of the number of control points / electric energy consumption, presented in Annexto this document.

For overhead networks, it is required to replace all non-insulated wires with a length of about 30 km for insulated wires (AsXSn 2x25mm) and install them independently of low-voltage subscribers.

Fixtures and accessories after disassembly must be settled with their owners : Commune of Chmielnik and PGE Dystrybucja SA

A detailed description of the subject of the order-Stage I contains:

1) Functional and Utility Program constituting Annex No. 5.1 to the ToR, taking into account introduced changes, deletions, additions.

Deletions do not apply to the conducted proceedings.

Stage I implementation is implemented with support under the Regional Operational Program of the Świętokrzyskie Voivodeship for 2014-2020, in under measure 6.2. Promotion of low-emission strategies and sustainable urban mobility - ZIT KOF.

b) Stage II - Modernization of lighting in the municipality of Chmielnik

The contractor is obliged to carry out a lighting modernization project covering all elements described in the attached functional and operational program for Area II of the Chmielnik commune and based on agreed technical project will perform the work planned in this venture. : disassembly of 557 existing pieces opraw equipped with discharge light sources with different degree of wear along with disassembly of accompanying equipment, among others booms, uninsulated conductors;

- assembly of 704 new LED luminaires, including assembly of 147 pieces of luminaires in a new location on existing poles of low voltage power network, as a supplement to road lighting and installation of new equipment-among others control and measurement boxes, insulated conductors and booms.

After disassembling the existing fittings there will be mounted fittings with LED sources. Install the boom in accordance with PFU. Optimization of the luminaire suspension geometry is achieved through the use of fittings with adjustable angle of inclination to obtain the best lighting effects, including compatibilitytrow with the PN-EN 13201 standard.

The task includes the elevation of control points and electricity settlement from the closed facilities of the Power Distribution System Operator, ie PGE Dystrybucja S.A. for positions that do not limit accessibility. The elevation of control points must take into account the Employer's guidelines for reducing the number of control points / energy consumption-electrical power, shown in the attachment to this document.

For overhead networks, it is required to replace all non-insulated wires with a length of 36 km for insulated wires (AsXSn 2x25 mm) and install them independently of the low voltage subscribers.

The fittings and accessories after disassembly must be settled with their owners: Chmielnik Commune and PGE Dystrybucja SA

Detaillead description of the subject of the Phase II order contains:

1) Functional and Utility Program constituting Annex No. 5.2 to the ToR, taking into account introduced changes, deletions, additions.

The deletions do not apply to the conducted proceedings.

4)Simplified plan for the placement of luminaires in the area of exchange for area II Stage II Annex No. 5.5 to the ToR.

5) Coverage map of the project Stage II Annex No. 5.7 to the Terms of Reference - Implementation of STAGE II is implemented with support under the Regional Operational Program of the Świętokrzyskie Voivodeship for 2014 2020, within the framework of measure 3.4. Low-emission strategy, balanced supportof multimodal urban mobility.

1. The ordering party requires the contractors to pay a bid bond in the amount of PLN 50, 000.00 (in words: fifty thousand PLN 00/100), which must be paid before the deadline for submission of tenders. The bid bond may be brought in one or several of the forms listed in art. 45 par. 6 Public Procurement Law, i.e.: a) money,

b) bank surety or surety.the savings and credit union, except that the surety is always a cash guarantee;

c) bank guarantees;

d) insurance guarantees;

e) sureties granted by entities referred to in art. 6b paragraph 5 points 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws No. 109, item 1158 with later amendments).

3. With warranty content and handrailsń referred to in the above point 2 lit. b-e of the SIWZ (Article 45 section 6 items 2-5) must result in an unconditional, irrevocable and first order written by the Employer, obliging the guarantor to pay to the Orderer the amount specified in the guarantee.

4. The deposit paid in cash must be paid to the bank account of the Ordering Party:

5. For effective payment of a bid bond inThe Employer will recognize the bid bond that will be placed on the Employer's account before the deadline for submitting bids.

6. Proof of payment in a form other than cash must be: - attach to the offer in the original.

- place in room No. 209 Municipal Office in Chmielnik, Kosciuszko Square before the deadline for submission of offers. The ordering party returns the bid bond to all contractors immediately after youthe bid of the best offer or annulment of the proceedings, with the exception of the contractor whose tender was selected as the most advantageous subject to Article 46 section 4a of the Public Procurement Law (ie if circumstances affecting the retention) will occur

8. The Contractor whose offer will be selected, the Employer will retain the bid bond with interest if: a) the Contractor has refusedsigning the contract on terms specified in the offer,

b) the contract has become impossible due to reasons attributable to the Contractor. c) the Contractor whose offer will be selected will not provide the required performance bond; d) the Contractor in response to the call for which speech mentioned in art. 26 par. 3 and 3a PZP, for reasons attributable to him, did not make any statements-or documents confirming the circumstances referred to in art. 25 ust. 1 PPL, statements referred to in art. 25a para. 1 PPL, powers of attorney or did not agree to correct the mistake referred to in art. 87 par. 2 point 3 of the PPL, which meant that it was not possible to select the offer submitted by the contractor as the most advantageous one.

9. The ordering party will return the bid bond to all contractors immediately afterthe most favorable offer or after the cancellation of the proceedings, with the exception of the contractor whose tender was selected as the most advantageous, subject to art. 46 ust. 4a Pzp. The deposit paid in the form of a transfer will be returned to the account from which it was received, unless the Contractor indicates another account number. The contractor whose offer has been selected as the most advantageous contracting party returns wadium immediately after the conclusion of the public procurement contract, subject to art. 148 para. 4 PPL.

The Employer does not specify the condition of participation in proceedings in the field of competences or entitlements to conduct a specific professional activity, as long as it results from separate regulations.
