The subject of the order is a comprehensive organization of the participation of the Lodz Voivode delegation in the trade fair event SLUSH 2018 in Helsinki, Finland, ktore will take place on 4-5.12.2018 (departure and return on 3-6.12.2018), as part of the "Lodzkie4business" project co-financed from the funds coming from the Regional Operational Program for the Lodz region for 2014-2020. Priority axis II. Innovative and competitive economy, II.2. Internationalization of enterprises, II.2.2. Economic promotion of the region; Task 2 System. Gospodarcza.

The subject of the order is a comprehensive organization of the participation of the Lodz Voivodship delegation at the trade fair SLUSH 2018 in Helsinki, Finland, which will take place on 4-5.12.2018 (departure and return on 3- 6.12.2018), as part of the "Lodzkie4business" project co-financed from the funds coming from the Regional Programfor the province of Lodz for the years 2014-2020. A detailed description of the subject of the order is attached as Annex 5 to the Terms of Reference. Contract draft and Annex No. 1 to the contract.

1. You can apply for the ordertenants who meet the conditions for participation in the proceedings regarding:

1.1. competence or entitlements to conduct a specific professional activity, if it results from separate provisions.

1.3. technical capacityprofessional training described below in item III.1.3). • Contractors who are not subject to exclusion and fulfill the conditions for participation in the procedure may apply for the order. The ordering party does not need to show grounds for no exclusion for subcontractors. The Contracting Authority does not provide for the exclusion of the Contractor in the cases specified in art. 24 sec. 5.

List of statements or documents, pconfirming fulfillment of the conditions for participation in the proceedings and lack of grounds for exclusion - In order to assess compliance with the conditions for participation in the proceedings and the lack of grounds for exclusion, referred to in art. 24 sec. 1 point 13-22 of the PPL Act, the Contractor is obliged to submit the following documents by the end of the deadline for submitting offers: - A uniform European order document (Annex No. 1 to the Terms of Reference). Information forworth in the above the document will constitute the preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the proceedings (Instructions for completing the document can be found at https://www.uzp.gov.pl/__data/assets/pdf_file/0015/32415/Instructions - filling -food-ESPD.pdf). 2. Ordering before the allowanceno orders, call the contractor whose offer has been rated the highest, to submit within the prescribed period (not shorter than 10 days), valid as at the date of submitting the following statements or documents:

2.1. confirming no grounds for exclusion from the proceedings:

a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Public Procurement Law, issued no-earlier than 6 months before the deadline for submitting bids. Note: In the case of contractors being collective entities, information about the collective entity should also be submitted;

b) the contractor's statement that the contractor has not been the subject of a final court decision or a final administrative decision about defaulting on tax payment, fees or social security or health insurance premiumsor in the event of such a verdict or decision, documents confirming payment of such claims together with any interest or fines or conclusion of a binding agreement regarding repayment of such debts; c) declaring the contractor about the absence of a ruling against him as a precautionary measure prohibiting applying for public orders,

2.2. If the contractor is established or-place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point: -2.1. point a) - provides information from the relevant register, or in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the contractor has its seat or place of residence has the person concernedinformation or document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law Act - If the country in which the contractor has a registered office or place of residence has a person to whom the document applies, no documents referred to in point 2.2 is replaced by a document containing the contractor's statement, indicating the person or persons entitled to-his representation, or a statement of the person whose document was to be submitted, submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the place of residence or place of residence of the contractor or place of residence of the person. the range of

3. Supplementary information to item IV.2.6 of the announcement: The offer validity period is 60 days.

4. Note: The ordering party in accordance with art. 24aa paragraph 1 of the Public Procurement Law, reserves the possibility of making the first cthe tendency to evaluate the offers, and then to examine whether the contractor whose tender was rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure.
