Supply of equipment and implementation of the permanent exhibition of the Sybir Memorial Museum

The subject of the order is the supply of equipment and the implementation of the permanent exhibition of the Sybir Memorial Museum. The exhibition will be located in the complex of the former military depot at ul. Węglowa w Białymstoku on the plot with the geodesign number 788/9. The building area is 3 705, 15 m2, usable area - 5, 458.10 m2, cubic capacity 28388, 00 m3, the number of storeys - 2 (ground floor), the building has a partial basement. The object is not located in the area of the municipal record of monuments, nor is it covered by conservation protection. The concept of arranging the permanent exhibition at the Sybir Memorial Museum was prepared by TEMPORA S.A., which will supervise the implementation of the exhibition.

Main Site: Białystok

1. Subject of the contractand there is the supply of equipment and the implementation of the permanent exhibition of the Sybir Memorial Museum. The exhibition will be located in the complex of the former military depot at ul. Węglowa w Białymstoku on the plot with the geodesign number 788/9. Building area is 3 705, 15 m2, usable area - 5, 458.10 m2, cubic capacity 28, 388.00 m3, number of storeys - 2 (ground floor), building with a basement. The object is notis located in the area of commune record of monuments, nor is it covered by conservation protection. The concept of arranging the permanent exhibition at the Sybir Memorial Museum was prepared by the company TEMPORA S.A., which will supervise the implementation of the exhibition.

2. The exhibition consists of:

Stage I: construction of exhibition stand divisions, delivery, assembly and start-up of multimedia installations- Stage II: preparation of scenarios and production of audiovisual materials, development of content for labels and translations, - Stage III: production and delivery including assembly of permanent exhibition elements, implementation of decor and accessories, - Stage IV: production of graphic elements, signatures / descriptions ( along with translations) and other work / supplies indicated in the documentation, preparation of documentation of the contractordescent,

3. The scope of the subject matter of the order, referred to in point 1, includes in particular:

1) elaboration:

a) content and narration of the exhibition and the placement of exhibits and other illustrative elements,

b) detailed film scenario, multimedia content in multimedia kiosks, animation, presentations, sound recordings,

c) graphic design of films, animations, and content-multimedia in multimedia kiosks and presentations,

d) multimedia control and lighting control system and multimedia content management.

2) performance based on the prepared by the Contractor and approved by the Employer for the implementation of specific scenarios and details projects:

a) films,

b) multimedia animations and presentations,

c) multimedia content in kiomultimedia,

d) sound recordings,

e) information system and visual identification.

3) performance, delivery and assembly based on detailed designs:

a) elements of scenography and interior design,

b) models, positions and artistic installations, multimedia,

c) teletechnical installations and electrical installations for the needs of multimedia and teletechnical equipment and devices, -lighting,

d) multimedia and lighting control system and multimedia content management,

e) multimedia and teletechnic devices and equipment,

f) electrical equipment for the installations listed above (including lighting fixtures and light sources)

4) delivery and installation with software launch and tests: for content management, AV devices, lighting, installationexposures.

5) preparation of as-built documentation.

4. A detailed description of the subject of the order is attached as Annex 8 to the ToR.

Not applicable

At the request of the Awarding Entity, the Contractor should submit: - information from a bank or cooperative savings and credit union confirming the amount of funds held or creditworthiness of the contractor in a period not earlier than 1 miSending before the deadline for submission of offers. The basics of exclusion

1. Contractors shall be excluded from the proceedings in the cases specified in art. 24 sec. 1 Public Procurement Law.

2. The contracting authority foresees the exclusion of the contractor based on art. 24 sec. 5 point 1 and items 5-8 of the Public Procurement Law

II. For the purpose of preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings, the contractorada:

1. Single European Order Document / ESPD (hereinafter referred to as "EAT") on or according to Attachment 2 to the ToR The document should be filled in accordance with the instructions, which can be found at www.uzp.gov.pl, tab: Repozytorium wiedzy / Jednolity Europejski Order Document Pre-prepared by the Ordering Party for the subject proceedings in electronic form (in the format-.xml - to be imported on the ESPD website) is available on the Ordering Party's website. Note: In Part IV EAT (Qualification Criteria), the Contractor may only fill in the section a.

EAT statement, the Contractor should send the Ordering Party in an electronic form with a qualified electronic signature, in accordance with the rules set out in Chapter XVI paragraphs 3-5 of the SIWZ.

3. Documents, in particular obligations of other entities, to provide the Contractor with the necessary resources for the needs of the order, referred to in Chapter XI point 2 point 2 of the ToR if it concerns

III. A list of statements or documents submitted on a summons to confirm the circumstances referred to in art. 25 ust. 1 point 3 of the PPL Act (regarding the offer of the mostabove rated):

1. The Contractor shall submit the documents listed in Chapter VIII point 1 items 1-9 of the ToR

2. Documents of foreign entities and contractors established in the territory of the Republic of Poland in relation to a person residing outside the territory of the Republic of Poland are specified in the executive regulation to the Public Procurement Law (Journal of Laws of 2016, item 1126). The contracting party demands from the contractor, which relies on the ability or situationother entities on the terms specified in art. 22a of the Public Procurement Law, present the documents in Sec. VIII point 4 of the SIWZ.

IV. Other documents:

1. In the case of contractors jointly applying for the order, in order to confirm that the contractor is not subject to exclusion and meets the conditions:

1) EAT, referred to in Chapter VII point 1 of the ToR and declarations and documents, oreferred to in Chapter VIII point 1 items 1-9 of the ToR shall be submitted by each contractor,

2) documents referred to in Chapter The IX SIWZ shall submit respectively a contractor who demonstrates fulfillment of the condition, in the scope and on the principles described in Chapter V point 1 items 2 and 3 of the SIWZ.

2. If the contractor, in order to confirm the fulfillment of the conditions, relies on the resources of other entities, the contractor is obliged to submit:

1) EAT on other subjects referred to in Chapter VII point 2 SIWZ in electronic form,

2) documents in writing together with the offer (in particular obligation of other entities to give the Contractor at the disposal of the necessary resources for the implementation of the order, Annex 5 to the Terms of Reference), which specify in particular:

a ) scope of available contractors resources of another entity,

b) mode of lectureusing the resources of another entity, by the contractor, while performing the public order, - c) scope and period of participation of another entity when performing the public order. - The contractor, which relies on the financial or economic situation of other entities, is jointly and severally liable with the entity that committed to provide resources, for damage sustained by the orderer arising as a result ofto access these resources, unless it is not at fault for not sharing the resources.

3. The Contractor, within 3 days from the date of publishing the information on the opening of offers on the website, shall provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law, along with other contractors who submitted bidsin this proceeding (attachment No. 6 to the ToR).
