3.1.5. Statement of compliance with obligations related to taxes and social security, environmental protection, employment protection and working conditions - Annex 9 is filled.

3.2. Pricenow-a proposal (prepared according to a model - appendix No 3), which is placed in an envelope marked "Suggested price parameters."

The evaluation of the unit prices proposed by the participant is formed in the way indicated in the instructions for participation. of the participant's bid is calculated as the sum of the individual estimates according to the following formula:
