The subject of the contract is the service entitled:" Supply, assembly and commissioning of ICT equipment along with the delivery, integration and implementation of "Front line" software and "Back office" to provide e-services "under the project:" Lubuski e-Urząd II ". The Lubuski e-Office II project, under whichThis contract is being implemented with co-financing from the European Regional Development Fund under the Lubuskie 2020 Regional Operational Program, Axis 2. Digital development, Measure 2.1. Development of the information society. with the delivery, integration and implementation of software"Front line" and "Back office" to provide e-services "under the project:" Lubuski e-Office II ". The Lubuski e-Office II project, under which this contract is carried out, is implemented with funding from the European Regional Development Fund under the Lubuskie 2020 Regional Operational Program, Axis 2. Digital development, Measure 2.1. Society developmentinformation.

2. The subject of the order was divided into the following stages:

- stage I of implementation preparation,

- stage II of system implementation.

3. The detailed scope and terms of the contract are specified in:

- Annex 1 to the ToR, detailed description of the subject of the contract,

- Annex 10 to the ToR, draft contract,

- Annex 12 to the ToR, feasibility study of the project: Lubuski e-Urząd II wera from the application stage.

Contractors who meet the conditions of participation referred to in Art. 22 paragraph 1b Pzp and are not subject to exclusion on the basis of Art. 24 paragraph 1 point 13 22 and par. 5 points 1, 2, 4 and 8 of the Public Procurement Law.

1. The Contractor shall attach to the offer, valid for the day of submission of offers, EAT.

2. In accordance with art. 26 section 1 of the Act, before awarding the contract, Zathe person calling will call the Contractor whose bid was highest rated to submit valid statements or documents as at the date of submission, i.e.:

1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids,

2) a certificate from the competent head of the tax office confirmingego that the Contractor is not in arrears with payment of taxes issued no earlier than 3 months before the deadline for submitting bids or requests to participate in the proceedings, or other document confirming that the Contractor has entered into an agreement with the relevant tax authority regarding the repayment of these debts along with possible interest or fines, in particular obtainedlegal release, deferment or installment of overdue payments or suspension of the entire implementation of the decision of the competent authority,

3) certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of insurance premiums social or sentencevalid, issued not earlier than 3 months before the deadline for submission of bids or requests to participate, or another document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular obtained by law, release, deferment or distribution intooverdue payments or withholding in full the decision of the competent authority,

4) an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or the register, in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the PPL Act. Note: The Ordering Party will download the above by itselfdocument, if the Contractor indicates, in the bid form, the Internet address of publicly available and free databases from which the required document can be downloaded, 5) the Contractor`s statement that he has not issued a final court judgment or the final administrative decision on arrears with payment of taxes, fees or contributions for social or health insurance or in the event of deliverysuch a judgment or decision of documents confirming the payment of these debts together with any interest or fines or conclusion of a binding agreement on the repayment of those debts,

6) the Contractor`s statement on the lack of a ruling against him as a preventive measure prohibiting him from applying for public procurement,

7) the statement Contractors not arrears in paymenttaxes and local fees referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2019, item 1170, as amended),

8) list of services rendered, and in the case of periodic benefits or continuous, also performed, within the last 5 years before the deadline for submission of bids for participation in the procedure, and if the period of business activity is shorter in this period together withwith the value of the services concerned, subject, dates of performance and entities,

9) list of persons referred by the Contractor to the implementation of the public contract, in particular those responsible for the provision of services,

10) documents indicated in Annex No. 1 to SOPZ, Annex No. 2 to SOPZ and annexes from A1 to A13 to SOPZ to confirm compliance with the offered deliveries and / orservice requirements specified by the Employer.

Other statements and documents.

1. "Offer form" in accordance with Annex 2 to the ToR. The contractor is required to include in the offer form the MD5 checksum of the virtual machine file (s) being a digital print of the file with the solutions offered (samples). The checksum is an element of the sample referred to in chapter XII paragraph 1 lit.C SIWZ. For each virtual machine file, and if the Contractor delivers more than 1 virtual machine, for all virtual machine files, the Contractor is required to indicate separately MD5 checksums.

2. Power of attorney if applicable. The power of attorney must be submitted in the form of an original or notarized certified copy with a secure electronic signature inverified with a valid qualified certificate.

3. Commitment if applicable.

4. Justification regarding the trade secret if applicable. Any information constituting a trade secret within the meaning of the Act of 16.4.1993 on combating unfair competition (Journal of Laws of 2019, item 1010, as amended), which the Contractor reserves as a trade secret, powanother to be submitted in a separate file with the simultaneous selection of the command "attachment constituting the secrets of the enterprise", and then together with the files constituting the explicit part compressed into 1 file (ZIP).

5. Portable disks (main and spare), constituting a sample of the offered deliveries, referred to in chapter XII, section 1, letter C of the ToR. Disks should be submitted within the time limit referred to in chapterXXI section 1, i.e. until 3.3.2020 until 11:00 at the headquarters of the Employer, i.e. the Marshal`s Office of the Lubuskie Voivodship Administrative and Economic Department, general office, ul. Podgorna 7, 65-057 Zielona Gora, POLAND. This sample will be part of the content of the offer and will not be supplemented under Art. 26 section 3 Public procurement law. No disk assembly (samples) requiredthis deadline will result in rejection of the offer pursuant to art. 89 paragraph 1 point 2 of the Public Procurement Law
