Supervision of project and documentation.

Development of design and formal and legal documentation related to the implementation of the following task: reconstruction of the s / c gas network with connections in the city center. Wierzawice, stage I.

The subject of the contract is to prepare project and formal and legal documentation related to the implementation of the followingmajor tasks:

Main Site: The area of activity of Polska Społki Gazownictwa Sp. z o.o. Zakład Gazownicza in Jasło.

The subject of the order is to prepare project and formal and legal documentation related to the implementation of the following task:

"Reconstruction of the s / c gas network with connections in the city. Wierzawice stage I ".

The object of the contract includes:

1. Obtaining excerpts and excerpts from the recordsland plots on which the planned gas pipeline and associated infrastructure will be located (to the extent necessary to carry out the task).

2. Making maps for design purposes.

3. Obtaining excerpts from the Local Spatial Development Plan or, in the absence thereof, obtaining a decision for the Employer to determine the location of the public purpose investment.

4. Yieldnot consent of the owners of all real estate (or persons legally sanctioned to dispose of the property) to enter the area and permanent occupation of the property for the purpose of investment together with consent for the necessary felling of trees and shrubs (if necessary).

5. Development of a construction project 5 sets (including 2 copies for the architectural and building administration body, 3 copies for the Orderiając) electronic version on CD. The subject study should at the same time fulfill the function of a construction project and an executive design, which, pursuant to the Building Law, is the basis for obtaining a building permit decision,

or

6. land development project with technical description for submitting works not requiring a building permit in 5 sets (including 2 copies. for the architectural and construction administration body, 3 copies for the Awarding Entity) electronic version on a CD. The subject study should at the same time fulfill the function of a construction design and detailed design, constituting the basis for obtaining a notification in accordance with the Building Law.

7. Preparation of investor cost estimates 2 copies for the Ordering Party.

8. Before submitting documentation toa coordination meeting (if necessary) should be agreed at the territorially relevant gas plant location of the route of the gas pipelines, shut-off and relief systems.

9. Obtaining all required opinions, operands, arrangements and decisions, including at a coordination meeting, necessary to obtain a valid building permit or a valid notification of a conversion.

10. Obtaining for and in the namePurchaser`s valid building permit or notification.

11. Preparation of high greenery inventory in required cases together with obtaining permission to remove trees or shrubs.

12. Preparation of a detailed summary of the distribution network, broken down by diameter and length.

13. Reporting the progress in accordance with the concluded contract.

14. The material scope was presentedin the Technical Conditions for the reconstruction and construction of the gas network No. PSGJA.ZMSZ.763A.109.666246.1.18 of 11.5.2018.

15. The locations of gas pipelines presented on map maps are approximate and the lengths of the proposed gas network may change, however the above cannot be the reason for the Contractor`s claims regarding the change in remuneration for the workmanshipContracts.

Contractors who can apply for a contract:

1. are not excluded,

2. meet the following conditions for participation in the proceedings:

a) have the competence or authority to conduct specific professional activities, provided that it results from separate provisions, the Employer does not specify this condition;

b) they are in an appropriate economic or financial situation, i.e.they are insured against liability in the area of business related to the subject of the contract for a guarantee sum of at least PLN 200, 000.00 (in words: two hundred thousand zlotys);

c) have technical or professional capacity, i.e.: last five years before the submission deadline, and if the period of business is shorter includingduring the period, they completed orders consisting in the development of 2 project documentation, which correspond to their type of project documentation being the subject of the order (also in the scope of infrastructure other than gas, it is permissible to make documentation in the field of water supply or pressure sewage system made of PE pipes) with a length of 30% of the length of the object orders.

Dysponuher or will have the following persons capable of performing the contract (these persons will participate in the implementation of the contract) who have the following experience and qualifications in total:

a) demonstrate that in the last 5 years before the deadline for submission of tenders, and if the period of conducting activity is shorter in this period, completed orders involving the development of 2 ddesign documentation that corresponds to the type of design documentation being the subject of the contract (also in the field of non-gas infrastructure, it is allowed to prepare documentation for the water supply or pressure sewage system of PE pipes) with a length of 30% of the length of the subject of the contract;

b) professional experience, including over the past 5 years before expirym deadline for submitting requests to participate in the procedure, experience of fulfilling the function of a designer with an installation specialty), at the implementation of at least one contract involving the development of a construction design and pipeline detailed design for which building permit decisions have been obtained.

1 . Wyko may apply for the contractpersons who:

a) are in an appropriate economic or financial situation, i.e. they have civil liability insurance in the scope of their activities related to the subject of the contract for a guarantee sum of at least PLN 200, 000.00 (in words: two hundred thousand zlotys).

Where information regarding the amount of insurance, contained in appropriate places in a uniformm, they will refer to foreign currencies, then the Employer will recalculate them according to the average exchange rate of NBP table A published on the day of sending the contract notice for publication in the Official Journal of the European Union. In the absence of publication of the exchange rate of this currency by the National Bank of Poland on the day referred to above, the Awarding Entity will apply the average exchange rate of this currency last announcedlaunched by the National Bank of Poland before the abovementioned day.

2. The Contractor, which relies on the economic or financial situation of other entities, is jointly and severally liable with the entity that has undertaken to make the resources available, for the damage suffered by the Employer as a result of failure to make these resources available, unless the Employer is not guilty of not providing the resources.

I will call the Contractor whose offer has been rated the highest to submit it within a designated period, not shorter than 10 days, valid on the day of submitting the statements or documents confirming the circumstances referred to in art. 25 paragraph 1 of the Act, it is:

1. information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act and, regarding convictionfor an offense against imprisonment, to the extent specified by the Employer pursuant to art. 24 paragraph 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting bids.

Subject to the content of art. 133 section 4 of the Act, i.e. the Contractor is not subject to exclusion in the case referred to in art. 24 paragraph 1 point 13 lit. d of the Act, and in the case referred to in art. 24 usst.1 point 14 of the Act, if the person referred to in this provision has been convicted of an offense listed in Art. 24 paragraph 1 point 13 lit. d of the Act;

2. a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with taxes, issued not earlier than 3 months before the deadline for submitting bids, or another document confirming that the Contractorentered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption, deferment or installment of overdue payments as provided for by law or withholding in full the execution of the decision of the competent authority;

3. a certificate from the appropriate organizational unit of the Social Insurance Institution or Kaces of Agricultural Social Insurance or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids, or other document confirming that the Contractor has concluded an agreement with the competent authority regarding the repayment of these receivables along with any interestme or fines, in particular he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision of the competent authority;

4. an excerpt from the relevant register or from the central register and information on business activities, if separate provisions require an entry in the register or the register, in order to confirm the absence of grounds for exclusionon the basis of art. 24 paragraph 5 point 1 of the Act;

5. statements referred to in points 9.19.6 9.19.10 of the ToR (Annex 2 to the ToR);

6. third party obligation to provide resources (if applicable);

7. policy, and in the absence of another document confirming that the Contractor is insured against civil liability in the scope of the business related to the subjectm orders at least for the amount of PLN 200, 000.00 (in words: two hundred thousand zlotys) to confirm the condition specified in point 7.1.2.2. TOR;

8. evidence that the services listed in the single document carried out within the last 5 years before the submission deadline, and if the business period is shorter in that period, have been properly performed;

9. powersreferred to in point 7.1.2.3 of the ToR, together with current certificates of belonging to the relevant Building Chamber.
