Preparation of the design documentation of the medium pressure gas pipeline with gas connections in the cities of Dzikie-Kolonia and Dzikie. gas connections in the cities of Dzikie and Dzikie-Cologne, voivodeship Podlasie.

The subject of the contract is the execution of a construction and executive design, as well as the author`s supervision of the gas pipeline and gas connections in the cities of Dzikie and Dzikie-Kolonia, voivodeship Podlasie.

The subject of the contract includes in particular:

1 Construction design, detailed design and author`s supervision for the medium pressure DN 9 gas pipelineWith a length of about 1910 m, DN 63 with a length of about 975 m, DN 40 with a length of about 440 m with 33 gas connections DN 25 in total length of about 235 m in Dzikie and Dzikie-Kolonia. A shut-off valve should be designed for the gas pipeline

2. Obtaining all opinions, permits and decisions in the name and on behalf of the Employer from the competent authority, obtaining the final building permit decision.

3.Frameof the contract, the Contractor will provide complete design documentation, a bill of quantities and a cost estimate for the work. Not subject to exclusion.

2. They meet the following conditions for participating in the proceedings.

3. The Contractor may confirm compliance with wathe terms of participation in the proceedings, where appropriate, and in relation to a specific contract, or part thereof, rely on the technical or professional capacity or financial or economic situation of other entities, regardless of the legal nature of the legal relations with it.

4. Contractors may jointly apply for the contract.

II. From the procedure for awarding the contract Zthe amelling authority excludes the Contractor on the basis of the conditions specified in art. 24 paragraph 1 point 12-23 Public Procurement Law and art. 24 paragraph 5 Public Procurement Law, subject to art. 133 section 4 Public Procurement Law and art. 24 paragraph 7-12 Pzp.

The assessment of compliance with the above conditions will be made in accordance with the formula "meets does not meet", based on the information contained in the single document and documents to be provided by the Contractor to confirm theconditions for participation in the proceedings.

III List of statements and documents to be provided by the Contractors to confirm compliance with the conditions for participation in the proceedings and the absence of grounds for exclusion.

Documents and statements submitted with the offer by all Contractors

Together with the offer constituting Annex No. 1 to the ToR, The Contractor submits a valid statement as at the date of submitting bidsspecified in the Terms of Reference in the form of a single document (Annex 2 to the Terms of Reference) subject to the requirements set out in points 8.2.1- 8.2.9 of the Terms of Reference and documents referred to in points 8.1.2- 8.1.6 of the Terms of Reference, i.e.

1.W in the case of contractors jointly applying for the contract (in the form of a consortium, civil partnership), the Single Document shall be submitted by each

of contractors jointly applying for the contracte in compliance with the requirements specified in points 8.2.1 - 8.2.9 of the ToR. These documents confirm the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion

To the extent that each of the contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion.

2. Contractor, which refers to the resources of other entities, in order to demonstrate the absence of a basis for themin exclusions and compliance to the extent that the conditions for participation in the proceedings are cited, also submits uniform documents regarding these entities, signed by the entity to which the documents relate - subject to the requirements specified in points 8.2.1 8.2.9 of the ToR .

The offer together with attachments, including a single document, should be submitted in electronic form with a qualified electronic signature, via the PSG Purchasing Platform.

The Employer informs that he will carry out the procedure in accordance with Article. 24 aa PZp, which means that it will first evaluate the bids and then examine whether the Contractor whose bid was assessed as the most favorable is not subject to exclusion and meetsconditions for participation in the proceedings.

The Employer does not describe how to assess this condition. shorter than 10 days, valid on the day of submitting the statements or documents confirmingthe circumstances referred to in art. 25 paragraph 1 of the Act, it is:

1. Information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act and, regarding conviction for an offense to imprisonment, to the extent specified by the contracting authority pursuant to Art. 24 paragraph 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting bids.

Z zaguarding the content of art. 133 section 4 of the Act, i.e. the contractor is not subject to exclusion in the case referred to in art. 24 paragraph 1 point 13 lit. d, and in the case referred to in art. 24 paragraph 1 point 14, if the person referred to in this provision has been convicted of an offense listed in art. 24 paragraph 1 point 13 lit. d.

2 Certificates from the competent head of the tax office confirming thatthe contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables along with any interest or fines, in particular he obtained the exemption provided for by law, deferment or installment arrearsh payments or withholding in full of the decision of the competent authority;

3 Certificates of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions issued not earlier than 3 months before the end ofthe time for submitting bids or other document confirming that the contractor has entered into an agreement with the competent authority regarding the repayment of these claims together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full the decision of the competent authority ;

4 Copy from the relevant register or fromcentral records and information on business activities, if separate provisions require an entry in the register or the register, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;

5. Contractor`s statements in accordance with Annex 4 to the ToR.

6. List of persons specified in point 6.1.2.3 lit. and the ToR, directed by the Contractor to execute the order, responsible for the provision of services, together with information on their professional qualifications, entitlements, experience and education necessary to perform the contract, as well as the scope of their activities, and information on the basis for their disposal, according to the template constituting Annex 5 to the ToR.

7.List of services rendered, referred to in point 6.1.1.3 lit. b SIWZ, pprepared according to the specimen constituting Annex 6 to the ToR.

The list should be accompanied by evidence stating that these services have been performed properly, the evidence referred to are references or other documents issued by the entity for which the services were performed, and if, for a justified reason of an objective nature, the contractor is unable to obtain these documentscontractor`s declaration

8. The proceedings are conducted in Polish.
