The subject of the orders is: Preparation of project documentation for the execution of construction works involving the reconstruction of gas networks with connections in the months of: Gdańsk ul. Chełmińska / Chwaszczyno ul.Sambora SG / 00000418]. "The subject of the contract includes in particular: preparation of design documentation for the construction of a gas network, provision of expert services on the subject and value of the order in each procurement procedure for the execution of construction works indicated in the project documentation, exercising author's supervision, registration of the Construction Log, one-offupdate the investor's cost estimate, obtain the final decision on the construction permit on the behalf and for the benefit of the Awarding Entity, and make cost estimates for investors. A detailed description of the subject of the order is specified in the model contract constituting Attachment 2 to the Terms of Reference and in the description of the subject of the order constituting Annex No. 1 to the contract.

The contractors who are allowed to order can apply:

1.1. they are not subject to exclusion,

1.2. meet the following conditions for participation in the proceedings:

1.2.1. they have competences or licenses to conduct a specific professional activity, if it results from separate regulations, ie the Ordering Party does not specify this condition.

1.2.2.zthey are in the economic and financial situation that ensures the execution of the order, but this condition will be fulfilled if the Contractor has an insurance policy documenting that the Contractor is insured against civil liability in the scope of activities related to the subject of the order for the sum insured of PLN 200, 000 gross

1.2 .3. they have these abilitiescharter or professional, i.e.

1.2.3.1. performed during the last three years before the deadline for submission of tenders, and if the period of business is shorter, during this period, at least three services consisting of execution, project documentation for a gas or water or sewage network or heating network with a projected network length of at least 743.75 m ... 1.2.3.2. It hasat least one person with design qualifications in the specialty of heating, ventilation, plumbing and heating networks, installations and devices, or corresponding valid building permits, which were issued on the basis of previously applicable regulations, possessing: a) at least 3 years of professional experience in designing in the above specialty,

b) current certificate of membership in the Chamber Construction Engineers,

2. Ordering, specifying the requirements in terms of possessed powers, permits demonstrating the disposal of persons meeting the conditions for the provision of cross-border services within the meaning of art. 20a of the Act of 15.12.2000 on professional self-government of architects and civil engineers.

The contracting authority may, at any stage, make progressment, that the contractor does not have the required capacities if the involvement of technical resources or - Professional contractors in other economic ventures of the contractor may have a negative impact on the implementation of the order. The Contractor may, in order to confirm compliance with the conditions of participation in the proceedings, in appropriate situations and with regard to a specific order, or parts of it, the fieldbe based on technical or professional abilities or the financial or economic situation of other entities, regardless of the legal nature of the legal relationships that it has with it.

4. The contractor, which relies on the abilities or situation of other entities, must prove to the awarding entity that while implementing the contract, it will have the necessary resources of these entities, in particular presenting the undertakinglinking these entities to giving them the necessary resources for the needs of the order. The commitment must specify:

4.1. the range of resources made available to the Contractor;

4.2. the manner of using the resources of another entity, by the Contractor, while performing the public order.

5. The Awarding Entity assesses whether technical or professional capacities or c. Provided by contractors by other entitiesh financial or economic situation, allow the contractor to demonstrate compliance with the conditions for participation in the proceedings and examine whether there is a ground of exclusion for that entity. The contractor, which relies on the economic and financial situation of other entities, is jointly and severally liable with the entity that has committed to provide resources for damage sustained by the ordererpermanent due to the lack of access to these resources, unless it is not at fault for failing to provide access to resources.

7. With regard to conditions regarding education, professional qualifications or experience, contractors may rely on the capacities of other entities if these entities provide services for which these capacities are required.

Are in an economic and financial situation ensureorder fulfillment, this condition will be met if the Contractor has an insurance policy documenting that the Contractor is insured against civil liability in the scope of activities related to the subject of the order for an insurance sum of at least PLN 200, 000 gross

1. Proceedings for the order in question are conducted in Polish.

2.The Contractor will be bound by the offer for 60 days. The period of being bound by the Offer starts as soon as the deadline for submission of tenders expires.

3. The offer with attachments and a single document under pain of nullity shall be prepared in electronic form and be accompanied by a qualified electronic signature, according to the requirements of the VWEC.

4. Offer with attachments and a single documentthey must be signed by the person / s authorized to represent the Contractor using a qualified electronic signature. Unless it results from other documents attached by the Contractor, the authorization (power of attorney) to sign the offer should be attached to the offer and made in electronic form and bearing a qualified electronic signature. In addition to the form oferty The Ordering Party requires submission of a Price Form. The Price Form will not constitute an offer and is of an auxiliary nature, it does not include the calculation of all unit prices, which make up the price included in the Bid Form. The Price Form will be an attachment to the contract

And the prices in it will be indicated for the Contractor binding during the implementation of the Contract.

6. If the Contractor plans to entrustPart of the orders of Subcontractors should also indicate in the Bid Form, which part of the contract is going to be entrusted to subcontractors and provide subcontractors.

7. According to art. 13 para. 1 and par. 2 of the General Regulation of the European Parliament and of the Council (EU) 2016/679 of 27.4.2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of personsof such data and the repeal of Directive 95/46 / EC (Dz.UE.L, from 2016 No. 119,

The detailed description of personal data protection is provided in the Model Contract .8 The Ordering Party, prior to the order being awarded, will call the Contractor whose offer has been the highest rated, to submit in the designated, not cutthan 10 days, the current date as of the day of submitting statements or documents confirming the circumstances referred to in art. 25 para. 1 of the Act, specified in point 8.19 SIWZ.

9. Documents or statements submitted on request, the Contractor shall provide the Ordering Party with the use of means of electronic communication, to the e-mail address specified in point 9.1. SIWZ.

Documents or declarationshall be submitted in the original in the form of an electronic document provided by their issuer with a qualified electronic signature or in the form of an electronic copy certified as a true copy by the Contractor. In the case of submission of a copy of the document or statement by the Economic Operator, the signature by the Economic Operator by a qualified electronic signature is equivalentwith a confirmation by the Contractor of an electronic copy of the document or a statement for compliance with the original. In the case when the contractor submits an electronic document or a statement in a format that compresses the data, affixing the file containing the compressed data with a qualified electronic signature is synonymous with the contractor's certification for compliance with the originalall electronic copies of documents or statements contained in this file, with the exception of copies certified by another contractor applying jointly to him for the order, by the entity on the capacity or situation of the contractor, or by the Subcontractor.
